BackgroundOver one million Korean night shift workers undergo clinical assessment of sleep using the Insomnia Severity Index (ISI) and Epworth Sleepiness Scale (ESS) by occupational medical examination each year. Therefore, this study was conducted to evaluate the reliability and validity of the ISI and ESS using occupational medical examination data.MethodsThe study subjects included 12,056 shift workers at an electronics company who underwent an occupational health examination about shift work in 2018. The evaluation of the ISI and ESS was performed using exploratory factor analysis (EFA) and confirmatory factor analysis (CFA).ResultsAccording to the results of the EFA, the ISI had a single-factor structure, while the ESS had a two-factor structure, which was inconsistent with the findings of previous studies. The results of the EFA of 15 items from the combined ISI and ESS suggested a three-factor structure, with one factor for ISI items and two factors for ESS items, while the results of the CFA suggested sufficient validity of the combination of the ISI and ESS for sleep evaluation.ConclusionThe results suggest that the ISI and ESS have sufficient reliability and validity to be used for occupational health examinations about shift work.

Background The association of the severity of clinical symptoms and level of functional performance with the degree of magnetic resonance imaging abnormalities in patients with lateral epicondylitis has not been fully elucidated. This study aimed to investigate the association between the degree of anatomical abnormalities by evaluating three-dimensional magnetic resonance imaging models of the common extensor tendon and clinical parameters in patients with lateral epicondylitis. Materials and methods A total of 61 patients (24 men and 37 women) with lateral epicondylitis were included in this study. 3-Tesla magnetic resonance imaging was performed for all patients, and clinical parameters, including pain visual analog scale score, Quick Disabilities of Arm, Shoulder and Hand questionnaire score, elbow range of motion, and demographic factors, were evaluated. The proportion of lesion volume of common extensor tendon was adopted for three-dimensional model analysis. To determine the factors associated with clinical parameters, univariate, and multivariate linear regression analyses were performed. Results The proportion of lesion volume of common extensor tendon was not associated with clinical parameters. Gender and muscle edema were independently associated with pain visual analog scale scores. However, demographic factors and magnetic resonance imaging abnormalities were not associated with the Quick Disabilities of Arm, Shoulder, and Hand questionnaire score or elbow range of motion. Conclusions The three-dimensional volumetric lesion size of common extensor tendon was not associated with clinical symptoms and functional performance in patients with lateral epicondylitis. The clinical parameters of lateral epicondylitis may be influenced by several factors.

Pharmacopuncture and joint movement manual therapy for post-traumatic phalangeal osteoarthritis

Introduction: Post-traumatic osteoarthritis (PTOA) is a type of osteoarthritis that develops after ligament injury, meniscus injury, or fracture. Currently, there is no specific treatment approved for PTOA. This report describes the case of a 38-year-old man who suffered from PTOA of the right second distal interphalangeal (DIP) joint after practicing judo. Patient concerns: He visited the author's clinic at 3 months after the onset of symptoms. Symptoms included pain, limited motion, and joint enlargement of the right second DIP joint. Diagnosis: Partial tear of the ulnar collateral ligament of the DIP was revealed by magnetic resonance imaging. As the symptoms appeared after the traumatic event, PTOA was diagnosed. Interventions: Intra-articular hominis placenta pharmacopuncture and joint movement manual therapy were performed on each visit. Altogether, 10 sessions were performed until the symptoms improved remarkably. Outcomes: Visual analogue scale score (VAS) for pain; Quick Disabilities of the Arm, Shoulder, and Hand score (QuickDASH); joint circumference; and range of motion showed improvements at the end of the treatment. VAS decreased from 8.4 to 0.4, QuickDASH decreased from 44 to 13, joint circumference decreased from 5.5 to 5.4 cm, and range of motion was almost recovered, which was measured by the photographs. Lessons: There are not enough studies on phalangeal joint PTOA and its treatment. This case suggests pharmacopuncture and joint movement manual therapy as treatment options for phalangeal PTOA.
