Validity of the CAGE questionnaire in hospital

Abstract: CAGE is a good screening tool for alcohol abuse or alcohol dependency. Given the frequent-and insufficiently diagnosed-alcohol problems among inpatients, CAGE is indicated as a first-line tool for screening for the most severe alcohol use disorders in hospital. It should ideally be used systematically. A positive reply to any of the first three items should alert the clinician and prompt further investigation.

At enrollment biologic evaluation of all patients was carried out according to routine procedures.…”

“…A number of studies have demonstrated good reliability, sensitivity (77-86%), specificity (93-94%), and validity of the CAGE to screen for alcohol disorders. 36,37 There are several strong and widely used instruments used to assess DSM-IV criteria for the diagnosis of substance dependence. For example, the Structured Clinical Interview for DSM-IV (SCID) and the Composite International Diagnostic Interview (CIDI) have been found to have excellent interrater reliability, good test-retest reliability, and superior validity to standard clinical interviews.…”

“…The CAGE questionnaire, used as an index of problem drinking in our study, was developed by Ewing [28] and is one of the most widely used alcohol screening questionnaires as an easy-to-use tool to identify severe alcoholism [43,44]. The ADH1B*1/*1 genotype has been positively associated with problem drinking as assessed by the Kurihama Alcohol Screening Test (KAST) [42], and individuals with a high KAST score have a higher risk for alcohol dependence [25,45].…”
