Validity of Secretin Stimulation Testing on Proton Pump Inhibitor Therapy for Diagnosis of Zollinger-Ellison Syndrome

Abstract: INTRODUCTION: Zollinger-Ellison syndrome (ZES) is characterized by gastrinoma-induced hypergastrinemia causing excessive gastric acid secretion. Secretin stimulation tests (SSTs) are required for diagnosis in the majority of patients. Two case reports suggest that proton pump inhibitors (PPIs) cause false SST results. Consequently, PPIs are discontinued to allow hyperchlorhydria to recur; however, uncontrolled acidity can cause life-threatening complications in those with underlying undiagnosed ZES… Show more

“…Traditionally this would require cessation of all PPI therapy for two weeks prior to the test. However, a recent retrospective non-inferiority study evaluating SST on PPI versus SST off PPI in patients with MEN1 found that SSTs off PPI were non-inferior for sensitivity, specificity, and positive predictive value (PPV) [ 5 ]. This suggests cessation of PPI two weeks prior to SST is unnecessary in aiding in the diagnosis of ZES in patients with MEN1 syndrome with a high pretest probability of ZES.…”

“…Classically, it has been advised to stop a proton-pump inhibitor prior to the stimulation test as it confounds the results. However, recent literature suggests this may be unnecessary [ 71 ]. Interestingly, the use of proton-pump inhibitors has also been shown to falsely elevate Cg-A levels, confounding that diagnostic test as well [ 72 ].…”
