Amyloid burden and sleep blood pressure in amnestic mild cognitive impairment

Objective: To determine whether cortical b-amyloid (Ab) deposition is associated with circadian blood pressure (BP) profiles and dynamic cerebral blood flow (CBF) regulation in patients with amnestic mild cognitive impairment (aMCI).Methods: Forty participants with aMCI were included in this study. Cortical Ab depositions were measured by 18 F-florbetapir PET and expressed as the standardized uptake value ratio (SUVR) relative to the cerebellum. Circadian BP profiles were measured by 24-hour ambulatory monitoring during awake and sleep periods. The dipping status of sleep BP (i.e., the percent changes from the awake BP) was calculated and dichotomized into the dipper ($10%) and nondipper (,10%) groups. Dynamic CBF regulation was assessed by a transfer function analysis between beat-to-beat changes in BP and CBF velocity measured from the middle cerebral artery during a repeated sit-stand maneuver.Results: Age was positively correlated with a greater Ab deposition in the posterior cingulate, precuneus, and mean cortex. Accounting for the age effect, attenuated reductions in sleep systolic BP were associated with higher levels of posterior cingulate SUVR. Consistently, the nondippers exhibited a higher SUVR in the posterior cingulate than the dippers. Transfer function gain between changes in BP and CBF velocity was diminished in the nondippers, and moreover those individuals with a lower gain exhibited a higher SUVR in the posterior cingulate. Alzheimer disease (AD) may begin decades before the manifestation of clinical symptoms, as revealed by b-amyloid (Ab) depositions in the cerebral cortex.1 Mounting evidence suggests a link between AD and the presence of cardiovascular risk factors such as elevated blood pressure (BP).2 Physiologically, normal BP maintains a proper brain perfusion, which provides appropriate oxygen and nutrient supply, and may also play a pivotal role for Ab clearance. Conversely, sustained elevations in BP can impose a substantial mechanical stress on the cerebrovascular beds and subsequently cause vascular remodeling and dysfunction. In daily life, BP fluctuates considerably and presents prominent circadian rhythms. 4 Thus, a snapshot measurement of BP in the routine clinic setting is unlikely to represent the overall impact of BP on the brain.5 Indeed, elevated BP not only while awake but also during sleep has been linked to brain structural deteriorations 6-8 and cognitive dysfunction. 9-12 Nevertheless, the potential effects of changes in BP circadian rhythms on brain Ab deposition are yet to be understood.

Background:The acute effect of low-intensity walking on blood pressure (BP) is unclear. Purpose: To determine if the acute use of a walking workstation reduces ambulatory blood pressure (ABP) in prehypertensive men and women. Methods: Ten prehypertensive adults participated in a randomized, cross-over study that included a control workday and a walking workstation workday. ABP was measured for 7 hour during the workday and for 6 hour after work. Results: Both systolic BP (SBP) (134 ± 14 vs. 137 ± 16 mmHg; P = .027) and diastolic BP (DBP) (79 ± 10 vs. 82 ± 12 mmHg; P = .001) were lower on the walking workstation day. Postwork hours (4:00 PM-10:00 PM), SBP (129 ± 13 vs. 133 ± 14 mmHg; P = .008), and DBP (74 ± 11 vs. 78 ± 13 mmHg; P = .001) were also lower on the walking workstation day. DBP load was significantly lower during the walking workstation day, with only 14% of the readings above 90 mmHg compared with 22% of the control day readings (P = .037). Conclusion: Accumulation of very-light-intensity physical activity (~2 METs) over the course of a single work day using a walking workstation may reduce BP burden in prehypertensive individuals.

PURPOSE Current offi ce blood pressure measurement (OBPM) is often not executed according to guidelines and cannot prevent the white-coat effect. Serial, automated, oscillometric OBPM has the potential to overcome both these problems. We therefore developed a 30-minute OBPM method that we compared with daytime ambulatory blood pressure.METHODS Patients referred to a primary care diagnostic center for 24-hour ambulatory blood pressure monitoring (ABPM) had their blood pressure measured using the same validated ABPM device for both ABPM and 30-minute OBPMs. During 30-minute OBPM, blood pressure was measured automatically every 5 minutes with the patient sitting alone in a quiet room. The mean 30-minute OBPM (based on t = 5 to t = 30 minutes) was compared with mean daytime ABPM using paired t tests and the approach described by Bland and Altman on method comparison. RESULTSWe analyzed data from 84 patients (mean age 57 years; 61% female). Systolic and diastolic blood pressures differed from 0 to 2 mm Hg (95% confidence interval, -2 to 2 mm Hg and from 0 to 3 mm Hg) between mean 30-minute OBPM and daytime ABPM, respectively. The limits of agreement were between -19 and 19 mm Hg for systolic and -10 and 13 mm Hg for diastolic blood pressures. Both 30-minute OBPM and daytime ABPM classifi ed normotension, whitecoat hypertension, masked hypertension, and sustained hypertension equally. CONCLUSIONSThe 30-minute OBPM appears to agree well with daytime ABPM and has the potential to detect white-coat and masked hypertension. This fi nding makes 30-minute OBPM a promising new method to determine blood pressure during diagnosis and follow-up of patients with elevated blood pressures. INTRODUCTIONT he Framingham and the SCORE (systematic coronary risk evaluation) risk functions, both developed to assess the risk of cardiovascular disease, are based on standardized offi ce blood pressure measurements (OBPMs).1,2 Despite guidelines that advocate the relevance of well-executed, standardized OBPM to prevent several forms of bias, 3,4 it is well known that most caregivers do not execute OBPM strictly according to these guidelines. 5,6 In addition, up to one-quarter of patients is prone to the white-coat effect (in which patients exhibit elevated blood pressure in a clinical setting but not in other settings), which infl uences cardiovascular risk profi ling as well. 7,8 This white-coat effect cannot be overcome by standardized OBPM.
