Background: Compromised iron status is important in restless legs syndrome pathophysiology. We compared the efficacy and tolerability of ferric carboxymaltose (single intravenous dose) versus placebo for restless legs syndrome treatment in iron‐deficient nonanemic patients. Methods: Patients with moderate to severe restless legs syndrome and serum ferritin < 75 μg/L (or serum ferritin 75‐300 μg/L and transferrin saturation < 20%) were randomized to ferric carboxymaltose (1000 mg iron) or placebo. Mean change difference between ferric carboxymaltose and placebo in International Restless Legs Syndrome Severity Scale score from baseline to week 4 was the primary end point; week 12 was a secondary end point. Results: Ferric carboxymaltose treatment (n = 59) led to nonsignificant improvement over placebo (n = 51) in International Restless Legs Syndrome Severity Scale score at week 4 (difference [95% confidence interval], ‐2.5 [‐5.93 to 1.02], P = 0.163), reaching significance by week 12 (‐4.66 [‐8.59 to ‐0.73], P = 0.021). Conclusions: In patients who responded to treatment, ferric carboxymaltose may require more time to stabilize restless legs syndrome than previously assumed. © 2017 The Authors. Restless Legs Syndrome in Hemodialysis Patients: Association with Calcium Antagonists

Uremia-related restless legs syndrome (RLS) is a known form of secondary RLS. This cross-sectional survey included patients (n = 82) on stable hemodialysis (HD; >3 months, Kt/V >1.2) who were iron-replete, free of neurodegenerative or psychiatric disorders, severe polyneuropathy and radiculopathy, not exposed to antipsychotics/antidepressants, and not severely anemic. Forty-nine (60%) were RLS ‘positive’, and 25 (31%) had severe/very severe symptoms (International Restless Legs Syndrome Study Group criteria). None had been diagnosed previously. In a multivariate analysis, the prevalence of RLS was higher in diabetic patients [vs. nondiabetics; prevalence ratio (PR) 2.32, 95% CI 1.50–3.60, p < 0.001] and those exposed to Ca²⁺ antagonists (vs. nonexposed; PR 2.02, CI 1.47–2.76, p < 0.001), and also increased with dialysis duration (PR 1.05, 95% CI 1.02–1.09, p < 0.001). Association of Ca²⁺ antagonists and RLS in uremic patients has not been reported previously and deserves further research.

Restless legs syndrome and cancer: an analysis in three independent studies

We report results from three independently conducted studies on the prevalence of cancer in restless legs syndrome (RLS) cases and non RLS cases. Already in 1955 Ekbom observed RLS in patients with subsequently detected cancer. Since then only three case reports and one article assessed the relationship between RLS and cancer patients undergoing chemotherapy. The observed proportion of RLS in these cancer patients (18.3%) was about twice as high as in general population studies. Two of our studies were general population based studies (Dortmund Health Study (DHS); Study of Health in Pomerania (SHIP)) and one was performed among members of the German and Swiss RLS patient organizations (Course of Restless Legs Syndrome-Study (COR-S)). Additionally we used data from the population based, regional cancer registry of the Muenster district for comparisons. Our data show that in all three studies the prevalence of cancer was higher in RLS cases than in non cases for both genders. In two of the studies the increased risk was in part explained by age. The data of the COR-Study revealed a statistically significant higher risk for cancer in RLS subjects compared to the population based data of the regional cancer registry of Muenster. Our results suggest a higher proportion of cancer among RLS cases compared to individuals without RLS. Given an RLS prevalence of about 5 to 10 percent in the general population, a need to rule out cancer in patients with a new onset of RLS, especially among the elderly patients is important.
