The International Classification of Functioning, Disability and Health (ICF) holds great promise for providing the rehabilitation disciplines with a universal language. However, the ICF is still highly complex and questions remain about its practicability. The aims of this review were to (i) identify how the ICF is integrated in the clinical activity of physiotherapists, (ii) discuss advantages and limits of the use of the ICF, and (iii) suggest further possibilities for implementing the ICF by physiotherapists. A literature search on Medline, Cochrane, CINAHL and PEDro (key words: ICF, physiotherapy, physical therapy, clinical application, clinical use) was carried out, together with a check for articles on Google Scholar. Of 155 identified articles, 22 were specific to physiotherapy. These articles described the utility of the ICF to facilitate decisionmaking for physiotherapists; to classify the evaluation of therapy outcomes; to ensure that all aspects of human functioning are represented while testing the effectiveness and the reliability of these outcomes; to structure the documentation of assessments and interventions, as well as to ameliorate the communication. ICF has a clinical relevance for physiotherapists, although the limited reliability of the qualifier system for the feasibility of its implementation was shown. The authors emphasize that the ICF is a tool that facilitates the decision of what to measure but not how to measure.

The International Classification of Functioning, Disability and Health (ICF) is intended to provide a framework for practitioners. A client-centred ICF-based assessment (ICF-A) was developed to be used by occupational therapists in problem identification. The aim was to evaluate the ICF-based assessment (ICF-A) focusing on the examination of its utility on the basis of occupational therapists´ perspectives regarding clinical relevance and potential for implementation. Eleven occupational therapists, most of whom worked in hospitals, performed in total 99 ICF-A based assessments and completed three self-reported questionnaires related to the utility of the ICF-A, resulting in a total of 121 questionnaires.Data were analysed using descriptive statistics and directed content analysis. The results from this initial testing of ICF-A showed that its clinical relevance was considered low since ICF-A included too many categories. In addition, the time needed to perform the assessments decreased during the ten assessment occasions. Furthermore, concerning its implementation potential, the client-centred approach in the ICF-A was rated as weak. The target group did not benefit from using ICF-A in a hospital context. Hence, an investigation of its utility among occupational therapists in community and primary care will be the next step in the development of the ICF-A.

The International Classification of Functioning, Disability, and Health (ICF) created by the World Health Organization provides both a framework and a classification comprehensively covering domains of function and disability in rheumatologic patients. The ICF can be used as a universal language understood by medical doctors, health professionals, researchers, patients, and other groups. It is based on an integrative biopsychosocial model of functioning. For its implementation in rheumatology and medicine in general, practical ICF-based tools such as the ICF Core Sets are necessary. These Sets, which were developed in a standardized scientific process, consist of the ICF categories that are most relevant for a specific group of patients, e.g. chronic patients with rheumatoid arthritis. In rheumatologic rehabilitation, patient problems, medical findings, treatment goals, and treatment concepts can be structured by applying the ICF, ICF Core Sets, and an ICF assessment sheet to patients. In outcomes research, ICF Core Sets can support the selection of relevant outcome domains.
