Validation of a Novel Electronic Health Record Patient Portal Advance Care Planning Delivery System

Abstract: BackgroundAdvance care planning allows patients to articulate their future care preferences should they no longer be able to make decisions on their own. Early advance care planning in outpatient settings provides benefits such as less aggressive care and fewer hospitalizations, yet it is underutilized due to barriers such as provider time constraints and communication complexity. Novel methods, such as patient portals, provide a unique opportunity to conduct advance care planning previsit planning for outpati… Show more

“…For developing better messaging, however, there is a need for more implementation studies identifying effective practices for using the patient portal as a means to bolster patient-physician communication during visits. There have been successful examples of this, including collecting and displaying patient-reported outcomes through the portal, and using the portal to facilitate advanced care planning discussions [ 12 , 38 , 51 - 53 ]. These strategies should be replicated in additional settings.…”

“…This read rate exceeds rates of similar ACP interventions with younger cohorts. 14,16 Despite the high read rate, the relatively lower use of the portal for ACP compared with article-based AD completion and in-person ACP discussions highlights the need for further evaluation of how and why older adults choose to engage in ACP, including through the patient portal. 20 Our early work described perspectives of early adopters of the ACP tools.…”

“…Use of patient portals has resulted in increased advance directives (ADs) in the EHR in a few studies. [13][14][15] For example, Tieu et al 16 conducted a study among primary care patients to test whether a motivational electronic message linked to an external ACP workbook, AD, and letter from the primary care provider (PCP), increased ADs in the EHR. The intervention group completed an AD 5.5% of the time compared with 2% in standard care.…”

“…Portals thus offer a particularly accessible platform for patients and care partners to learn about COVID-19 treatment options and communicate goals of care with their providers. Over the last 5 years, our research team has partnered with health care system information technology experts, patients, caregivers, and providers to develop, test, and implement patient portal advance care planning strategies [ 20 - 24 ]. Specifically, the UCHealth patient portal offers patients the ability to learn more about advance care planning through web-based resources and provides the ability to create an electronically signed MDPOA form to choose a medical decision maker.…”
