The Tinnitus Handicap Inventory (THI) is widely used in clinical practice and research as a three-dimensional measure of tinnitus severity. Despite extensive use, its factor structure remains unclear. Furthermore, THI can be considered a reliable measure only if Cronbach's alpha coefficient and Classical Test Theory is used. The more modern and robust Item Response Theory (IRT) has so far not been used to psychometrically evaluate THI. In theory, IRT allows a more precise evaluation of THI's factor structure, reliability, and the quality of individual items. Method There were 1115 patients with tinnitus (556 women and 559 men), aged 19-84 years (M = 51.55; SD = 13.28). The dimensionality of THI was evaluated using several models of Confirmatory Factor Analysis and an Item Response Theory approach. Exploratory non-parametric Mokken scaling was applied to determine a unidimensional and robust scale. Several IRT polytomous models were used to assess the overall quality of THI. Results The bifactor model had the best fit (RMSEA = 0.055; CFI = 0.976; SRMR = 0.040) and revealed one strong general factor and several weak specific factors. Mokken scaling generated a reliable unidimensional scale (Loevinger's H = 0.463). In order to refine THI we propose that five items be removed. The IRT Generalized Partial Credit Model generated good parameters in terms of item location (difficulty), discrimination, and information content of items.

AbstractPurposeTo provide physicians and patients with the tools needed to evaluate patients’ problems and health-related quality of life by cross-culturally adapting and validating the Chronic Otitis Media Questionnaire 12 (COMQ-12), the Dizziness Handicap Inventory (DHI), the Neuropsychological Vertigo Inventory (NVI) and the Tinnitus Handicap Inventory (THI).Materials and methodsCOMQ-12, DHI, NVI and THI were translated into the Slovenian language and completed by patients treated at our department for chronic otitis media, vertigo or tinnitus. The control group for each questionnaire consisted of healthy volunteers. Internal consistency, test-retest reliability, discriminant validity, diagnostic accuracy and cut-off value were determined for each questionnaire.ResultsTest-retest reliability was excellent for DHI (ICC A=0.946) and NVI (p=0.315, ICC A=0.975), good to excellent for COMQ-12 (p=0.680, ICC A=0.858) and satisfactory for THI (p=0.120). Discriminant validity was confirmed for each questionnaire (p>0.05) using the Mann-Whitney U test (COMQ-12, DHI, THI) or the Welch t-test (NVI). COMQ-12 had acceptable (α=0.796) and DHI (α=0.910), NVI (α=0.950) and THI (α=0.924) perfect internal consistency. COMQ-12 and DHI had excellent, NVI acceptable and THI perfect diagnostic accuracy (AUC=0.987, AUC=0.999, AUC=0.781 and AUC=1.000 respectively). Cut-off values determined by Youden’s index were 7, 7, 9 and 56 for COMQ-12, THI, DHI and NVI, respectively.ConclusionSlovenian COMQ-12, DHI, NVI and THI are a valid and accurate tool for the diagnosis and measurement of health-related quality of life in patients with chronic otitis media, vertigo and tinnitus. They could aid general practitioners, occupational health specialists, neurologists and otorhinolaryngologists.

Background: Tinnitus may be objective or subjective. Subjective tinnitus is the type most commonly associated with psychosocial co-morbidities. This study was conducted with an aim to assess the severity of tinnitus and to know its correlation with anxiety and depression in adult population of Western Gujarat region.Methods: This prospective observational study was conducted in a tertiary care teaching hospital of Western Gujarat from December 2016 to July 2017. Patients with tinnitus were screened for anxiety and depression using tinnitus handicap inventory (THI, validated in Gujarati language), generalized anxiety disorder (GAD-7) and patient health questionnaire (PHQ-9). Severity of anxiety and depression was further evaluated using The Hamilton anxiety rating scale (HAM-A) and the Hamilton rating scale for depression (HRSD, also called HAM-D) respectively. Results: 68 patients fulfilled the inclusion criterion. Maximum patients (30.9 %) had moderate tinnitus, followed by severe (26.4 %), catastrophic (19.1 %), mild (16.2 %) and slight (7.4 %). There was a strong and significant correlation between the average THI scores and average GAD-7 scores (r=0.9708, p<0.00001) and average THI scores and average PHQ-9 scores (r=0.9906, p<0.0001) suggesting that patients with tinnitus were very likely to have anxiety as well as depression. However, irrespective of the severity of tinnitus, most patients had only mild anxiety (r=0.9403, p<0.00001) and mild depression (r=0.6758, p=0.001).Conclusions: The patients with tinnitus are very likely to suffer from mild anxiety as well as mild depression.
