Validated Scales for Colon Cleansing: A Systematic Review

Abstract: To conclude, all the published scales displayed limitations. The BBPS is the most thoroughly validated scale and should be used in a clinical setting. Between-scale comparisons for repeat colonoscopy time interval, ease, and pertinence of use for auditing are needed.

“…Regarding colonic cleanliness, there was consensus on the use of validated scales, [17][18][19][20] preferably segmental, with evaluation made only after complete washing and rinsing of fecal residues. As expected, the above-mentioned characteristics tended to favor the Boston Bowel Preparation Scale, 21 as its segmental and total scores are to be obtained after cleansing and rinsing. 19 Of note, the use of such a scale has recently been associated with the miss rate of relevant lesions.…”

“…The quality of colon preparation was graded using the Aronchick categorical measure ( i.e ., excellent, good, fair, poor and unsatisfactory) . We used Aronchick as the most widely used bowel prep scale in such studies . However, upon conclusion of our study, recent evidence supports Boston Bowel Preparation Scale to be superior in assessing the efficacy of bowel preparation .…”

“…The primary outcome was the success rate of bowel preparation, which was defined as an Aronchick score ≤3 (1, excellent; 2, good; 3, fair; 4, inadequate; and 5, poor) or a modified Boston bowel preparation scale (mBBPS) score that is revised based on the original Boston bowel preparation scale (BBPS) score for each residual colonic segment (≥2). The Aronchick score is used to assess the global quality of bowel preparation according to the residual liquid and proportion of visible intestinal mucosa, which was used before cleansing and suctioning . The original BBPS is a four‐point scoring system used after optimal cleansing and suctioning to assess the three segments of the colon: the right colon (cecum and ascending colon), the transverse colon (including the hepatic and splenic flexures) and the left colon (descending colon, sigmoid colon and rectum) .…”
