Enteroatmospheric fistulas (EAF) are rare but challenging and morbid complications of abdominal surgery and require time- as well as resource-consuming management. Furthermore, they severely affect patients' quality of life. Several treatment modalities for EAF management are described in the literature. We describe 3 consecutive cases of EAF treatment by employing negative pressure wound therapy (NPWT) along with either a special silicone fistula adapter or a Silo-Vac-like system in another case to isolate the fistula from the remaining abdominal wound. Spontaneous fistula closure was achieved in 2 of the 3 cases, and surgical resection of the small bowel segment harbouring EAF opening was possible in a third case after wound conditioning. The rate of fistula closure was 100% (n = 3/3). Compartmentalisation of the contaminated area using NPWT accelerated healing of the open abdominal wound remarkably. In summary, we present a useful tool for the challenging management of EAF and review the literature on different treatment options of EAF available today.

INTRODUCCIÓN:El abdomen abierto es una estrategia quirúrgica que ha permitido realizar un manejo quirúrgico de infecciones intraabdominales; una de las patologías intraabdominales que puede ser manejada como abdomen abierto es la diverticulitis. El manejo de la diverticulitis con un cuadro clínico séptico es quirúrgico y se pueden requerir de terapias que no son de aplicación frecuente.CASO CLÍNICO: Se trata de un paciente de sexo masculino de 46 años de edad que luego de ser diagnosticado con diverticulitis y tras una serie de intervenciones tuvo un desenlace con abdomen abierto, fistula biliar, ileostomía, colostomía, estado séptico y desnutrición severa. EVOLUCIÓN:El paciente fue manejando en forma integral, para el tratamiento quirúrgico se recurrió a la técnica de presión negativa (TPN) o sistema «vaccum», requirió varias cirugías para cambio del sistema vaccum y revisión de ostomías; el cierre definitivo de cavidad se realizó junto con el servicio de Cirugía Plástica. CONCLUSIÓN:El manejo adecuado del cuadro abdominal sin duda fue la clave para la recuperación del paciente, la utilización de técnicas innovadoras y con fundamento científico asegura una evolución adecuada del paciente. *DESCRIPTORES DeCS

The objective of this study was to describe the Brazilian scientific reports published over the past twenty years on Fournier's gangrene. It is an integrative review conducted through incursions in the LILACS, MEDLINE, BDENF and Coleciona SUS databases with the descriptor 'Fournier's Gangrene'. Texts published between January 1994 and July 2014, in Portuguese, and freely available online were selected. According to the 14 articles found, the main symptoms of Fournier's gangrene include discomfort with painful sensations, high fever, edema, malaise, and sweating. In most cases erythema and blistering are observed, evolving into a wound. The clinical profile assists in the selection of empirical antimicrobial therapy even before the culture results. It was concluded that early diagnosis and appropriate and aggressive treatment are crucial in patient prognosis. There is still a shortage of Brazilian scientific production with a high power of evidence on the subject. Descriptors: Fournier Gangrene; Wounds and Injuries; Therapeutics; Review. Objetivou-se descrever os registros científicos brasileiros publicados nos últimos vinte anos acerca da gangrena de Fournier. Trata-se de uma revisão integrativa realizada por intermédio de incursões nas bases LILACS, MEDLINE, BDENF e Coleciona SUS com o descritor "Gangrena de Fournier". Foram selecionados os textos publicados entre janeiro de 1994 e julho de 2014, em português e disponíveis gratuitamente online. De acordo com os 14 artigos encontrados, os principais sintomas da gangrena de Fournier incluem desconforto com sensações dolorosas, febre elevada, edema, mal estar e sudorese. Na maior parte dos casos são observados eritema e formação de bolhas, evoluindo para uma ferida. O quadro clínico auxilia na seleção da terapêutica antimicrobiana empírica antes mesmo do resultado da cultura. Conclui-se que o diagnóstico precoce e o tratamento adequado e agressivo são determinantes no prognóstico do paciente.
