Vaccine induced thrombotic thrombocytopenia: development and reactivity of anti-platelet factor 4 antibodies and immune pathogenic mechanisms

Abstract: In ultrarare cases, patients vaccinated with DNA adenovirus vector vaccine against severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2), develop a vaccine-induced immune thrombotic thrombocytopenia (VITT), with a high incidence of fatal cases. The causative agent is the development of platelet factor 4 (PF4)-dependent antibodies that resemble heparin-induced thrombocytopenia (HIT) complication, although many differences can be noticed in clinical presentation, antibody reactivity, involved epitopes on … Show more

“…Others could be more specific and linked to the viral strategy to infect human cells, through its specific binding to ACE2 [102] , [103] , [104] . The mechanisms underlying the induction of these various autoantibodies in certain patients could involve the many possibilities including antigen mimicry, and epitope spreading, like in most acquired autoimmune complications, as recently reviewed for VITT [117] .…”

“…The inducing primary cause is not always clearly identified. This can result from an abnormal exposure to PT due to a genetic predisposition (HLA system), and the immune system lacks the efficiency to select self-components from the non-self-components, or to correctly recognize altered self-components [85,86].…”
