Uveal Melanoma: A Review of the Literature

Abstract: Melanomas affecting different components of the uvea occur with differing frequencies and clinical presentations. Uveal melanoma is diagnosed via funduscopic exam and ancillary tests. These lesions may present with visual findings or incidental findings on physical exam. Metastasis occurs in approximately half of all patients with primary uveal melanoma. The liver is the most common site of metastasis. Enucleation was at one time considered the definitive local treatment for primary uveal melanoma, but has bee… Show more

“…Despite a good control of the localised tumour by radiotherapy or surgery, up to 50% of UM patients develop metastases mainly in the liver. Once the disease has spread, the prognosis is poor with median survival of 9 months, and the treatment is challenging with absence of efficient therapeutic strategies [2,3].…”

“…At present, no effective treatment exists to prevent metastasis, but early detection and intervention could be critical for a positive long-term survival outcome in UM. 143 , 144 Metastatic UM responds poorly to chemotherapy, targeted therapy, and immunotherapy. Nevertheless, unlike cutaneous melanoma, UM metastases generally do not respond to immune-checkpoint inhibitors.…”

“…These latter have an elective tropism for the hepatic parenchyma, so that the liver represents the primary site of metastatization in more than 90% of cases and about 50% of these patients have purely liver metastases [ 59 – 61 ]. Without any therapy, the median overall survival of patients affected by uveal melanoma with hepatic metastases is < 6 months [ 62 ]. Therefore, the therapeutic management of liver metastases from uveal melanoma represents, even today, an unresolved issue.…”
