ResumenLa micosis fungoide es un linfoma cutáneo de células T de etiología desconocida que afecta principalmente a la población adulta. En sus fases iniciales la fototerapia sigue siendo una de las principales opciones terapéuticas. La terapia con NB-UVB en estadios tempranos IA-IB en parches y placas delgadas ofrece ventajas sobre el PUVA, debido a su mejor tolerancia y menores efectos secundarios; la terapia PUVA sigue siendo la elección en micosis fungoide en placas de mayor grosor. En cuanto a la terapia de mantenimiento se plantea que podría estar asociada a mayor radiación acumulada, sin lograr prevenir futuras lesiones, siendo el antecedente de recaída el principal factor predisponente para presentar nuevas lesiones en el futuro.Palabras clave: Micosis fungoide, Fototerapia, PUVA, UVB, Tratamiento de la micosis fungoide. AbstractMycosis fungoides (MF) is a cutaneous T-cell lymphoma of unknown etiology that primarily affects the adult population. In early stages of the disease phototherapy is a major therapeutic option. At the time NB-UVB therapy in early stages IA-IB patchy and thin plaques offers advantages over PUVA, due to its better tolerance and fewer side effects, the PUVA therapy remains the choice to MF in thinner plaques. The maintenance therapy is posed that could be associated with higher accumulation of radiation without achieving prevents future lesions, being the history of relapse the main predisposing factor to present new lesions in the future.

Prognostic significance of the therapeutic targets histone deacetylase 1, 2, 6 and acetylated histone H4 in cutaneous T-cell lymphoma Aims: Aberrant histone acetylation has been associated with malignancy and histone deacetylase (HDAC) inhibitors are currently being investigated in numerous clinical trials. So far, the malignancy most sensitive to HDAC inhibitors has been cutaneous T-cell lymphoma (CTCL). The reason for this sensitivity is unclear and studies on HDAC expression and histone acetylation in CTCL are lacking. The aim of this study was to address this issue. Methods and results:The immunohistochemical expression of HDAC1, HDAC2, HDAC6, and acetylated H4 was examined in 73 CTCLs and the results related to histological subtypes and overall survival. HDAC1 was most abundantly expressed (P < 0.0001), followed by HDAC2; HDAC6 and H4 acetylation were equally expressed. HDAC2 (P = 0.001) and H4 acetylation (P = 0.03) were significantly more common in aggressive than indolent CTCL subtypes. In contrast, no differences were observed for HDAC1 and HDAC6. In a Cox analysis, elevated HDAC6 was the only parameter showing significant influence on survival (P = 0.04). Conclusions: High expression of HDAC2 and acetylated H4 is more common in aggressive than indolent CTCL. HDAC6 expression is associated with a favorable outcome independent of the subtype.

ABSTRACT. Narrow-band UVB (NB-UVB) is light over a very narrow band of wavelengths (around 311 nm) that is concentrated in the therapeutic range and minimally in the sunburn range. It has therefore become the phototherapy treatment of choice for skin diseases. The minimal erythema dose (MED) on canine skin for standardizing dosage schedules in NB-UVB treatment and histopathological analyses were performed in these dogs. In all 32 dogs tested, the MED ranged from 432 to 864 mJ/cm 2 . There were no significant differences in MED among breeds, sex and age groups. Histopathology obtained from areas irradiated by MED showed only mild vascular dilatation. These findings might be valuable for the application of NB-UVB phototherapy to canine skin diseases. Ultraviolet (UV) is invisible light that is part of the energy that comes from the sun [1]. It is made up of three types of rays from the longest wavelength to the shortest: UVA, UVB and UVC. Prolonged exposure to the sun causes skin damage and sometimes skin cancer because of these UV rays. However, scientists have found that although the different wavelengths of light can have adverse effects, they can also be used as UV phototherapy to treat skin diseases [6,13]. UV phototherapy is exposure to intense UV light sources of a specific wavelength or narrow range of wavelengths for specified amounts of time to treat a variety of skin disorders [14,17]. In human medicine, UVA and UVB irradiation has been used successfully to treat a number of diseases, including psoriasis, cutaneous T-cell lymphoma and atopic dermatitis [10,15,16,18].In human medicine, phototherapy devices use either short-wavelength broad-band UVB rays or the long rays of UVA. Broad-band UVB bulbs emit light in a broad range that includes the therapeutic wavelengths specific to the treatment of skin disorders but also is responsible for causing sunburn, which increases the risk of skin cancer. This limits the usefulness of therapeutic broad-band UVB [1]. In contrast, narrow-band UVB (NB-UVB) emits light over a very short range of wavelengths concentrated primarily in the therapeutic range [9]. In addition, NB-UVB phototherapy induces apoptosis of dermal T lymphocytes [12]. Thus, NB-UVB is considered effective for controlling human T-cell-mediated skin diseases such as cutaneous T cell lymphoma or atopic dermatitis [3,7,8,11].The minimal erythema dose (MED) is the minimum dose of radiation that produces perceptible skin erythema, and it is expressed in Joules per square centimeter (J/cm 2 ). The MED must be determined before UV therapy in order to minimize the inflammation of irradiated skin and define the initial irradiation dosage.
