Utilization of the Organ Care System Lung for the assessment of lungs from a donor after cardiac death (DCD) before bilateral transplantation

Abstract: In this manuscript, we present the first experience of evaluating donation after circulatory death (DCD) lungs, using the normothermic preservation Organ Care System (OCS) and subsequent successful transplantation. The OCS could be a useful tool for the evaluation of marginal lungs from DCD donors as it allows a proper recruitment and bronchoscopy in such donations in addition to continuous ex-vivo perfusion and assessment and treatment during transport. The OCS could potentially be a standard of care in the e… Show more

“…A lactate level below 5 mmol/l at the end of the perfusion period is considered to be the key criterion for organ transplantation. Gas exchange is a fundamental indicator in lung EVNP, and in the liver the production of bile is the important parameter. Reliable and rapid techniques to assess the quality and suitability of a kidney are also essential to ensure the safe expansion of the marginal kidney donor programme.…”

“…Recently, Mohite et al published a successful case report using the OCS TM Lung for the preservation and assessment of a DCD donor lungs who died 6 min after WLST. Donor criteria were not documented (56).…”

“…More recently, an integrated and transportable machine perfusion system, the Organ Care Support (OCS) Lung (Transmedics, Andover, MA, USA), has been used for lung assessment and reconditioning. The Harefield Hospital group presented the first experience with OCS in the DCD setting: they noted that this system allows for continuous ventilation, recruitment manoeuvres and further bronchoscopic evaluations (37).…”
