IntroductionEnhancing the efficiency of primary healthcare services is essential for a populous and developing nation like China. This study offers a systematic analysis of the efficiency and spatial distribution of primary healthcare services in China. It elucidates the fundamental landscape and regional variances in efficiency, thereby furnishing a scientific foundation for enhancing service efficiency and fostering coordinated regional development.MethodsEmploys a three-stage DEA-Malmquist model to assess the efficiency of primary healthcare services across 31 provincial units in mainland China from 2012 to 2020. Additionally, it examines the spatial correlation of efficiency distribution using the Moran Index.ResultsThe efficiency of primary healthcare services in China is generally suboptimal with a noticeable declining trend, highlighting significant potential for improvement in both pure technical efficiency and scale efficiency. There is a pronounced efficiency gap among provinces, yet a positive spatial correlation is evident. Regionally, efficiency ranks in the order of East > Central > West. Factors such as GDP per capita and population density positively influence efficiency enhancements, while urbanization levels and government health expenditures appear to have a detrimental impact.DiscussionThe application of the three-stage DEA-Malmquist model and the Moran Index not only expands the methodological framework for researching primary healthcare service efficiency but also provides scientifically valuable insights for enhancing the efficiency of primary healthcare services in China and other developing nations.

AimTo examine the prevalence and potential risk factors of multimorbidity among older adult in China. In addition, we investigated the pattern of multimorbidity.MethodsThis study is based on data from the fourth Sample Survey of the Aged Population in Urban and Rural China (SSAPUR) in 2015, a comprehensive survey of individuals aged 60 years or older in China. We calculated baseline data and prevalence rates for comorbidities, stratified by household registration, age, sex, education, exercise, and health insurance. Univariate and multivariate logistic regression analyses were conducted to identify potential risk factors for comorbidities. Furthermore, we determined the prevalence rates for the three most frequent disease combinations.ResultsA total of 215,040 participants were included in our analysis. The prevalence of multimorbidity was 50.5% among the older adult in China. The prevalence rate was slightly higher in rural areas than in urban areas, with rates of 51.5 and 49.6%, respectively (p < 0.001). Moreover, the prevalence rate was higher in females than in males, with rates of 55.2 and 45.3%, respectively (p < 0.001). Multivariate logistic regression analysis revealed that individuals aged 70–79 years (OR:1.40, 95% CI: 1.38–1.43, p < 0.001) and over 80 years (OR:1.41, 95% CI: 1.38–1.45, p < 0.001) had a higher prevalence of multimorbidity than those aged 60–69 years. The most prevalent pair of comorbidities was hypertension and osteoarthropathy, with 19.6% of the participants having these two conditions, accounting for 5.4% of the total participants.ConclusionOur findings indicate a high prevalence of multimorbidity among the older adult in China. Increased expenditure on preventive health care, popularization of general medicine and popular medical education may be adopted by the Government to cope with the high prevalence of multimorbidity.

Smoking, drinking, sleeping and risky sexual behaviors: a cross sectional study in China (Preprint)

BACKGROUND General lifestyle health behaviors (e.g., drinking, smoking) and risky health behaviors are important causes of death, and often have concomitant and aggregative characteristics. However, the associations between general lifestyle health behaviors and risky sexual behaviors are unclear. OBJECTIVE We aim to identify the associations between risky sexual behaviors and general lifestyle health behaviors. METHODS We employed a self-designed structured questionnaire to collect data in January 2022 through Credamo. The questionnaire mainly included the following three parts: the social-demographic characteristics, general lifestyle health behaviors and risky sexual behaviors. To control the confounding factors, propensity score matching (PSM) was used to match the people with or without risky sexual behaviors. Moreover, binary logistic regression was used to analyze the influencing factors of people with or without risky sexual behaviors. RESULTS In total, 3420 participants were incorporated in the data analysis. The matched data showed no significant differences between people with risky sexual behaviors and without in gender, age, marital status, nationality, religion, household registration, educational level and annual income (P>0.05). The prevalence of smoking(50.9% vs 26.8%), drinking(39.9% vs 25.6%), insomnia (25.9% vs 15.6%) was all higher among people with risky sexual behaviors than those without sexual behaviors. The rate of smoking everyday was 31.3% among people with risky sexual behaviors and 10.1% among those without risky sexual behaviors. The percentage of excessive drinking among people with risky sexual behaviors (30.5%) was significantly higher than that among people without risky sexual behaviors (17.1%). About time to fall asleep, the proportion of people who had risky sexual behaviors fall asleep after 12 o'clock was also higher(31.6% vs 20.5%) than their counterparts. Binary logistic regression showed that whether smoking, drinking alcohol, and insomnia were significant influencing factors for having risky sexual behaviors(P<0.05). CONCLUSIONS Smoking, alcohol consumption and sleeping problems were associated with risky sexual behaviors. Interventions regarding smoking cessation, moderate drinking and early to bed are important, which may be linked with reduced risky sexual behaviors.
