Introduction: Most of the surgeries done in our country under anesthesia are mostly at urban region and very low number at remote areas. Even those surgeries done under anesthesia do not receive proper premedication. Pre-anesthetic medicine has been linked to a variety of outcomes, including postoperative recovery time, the need for postoperative analgesia, and hospital stay. Yet, we have very less data regarding the patterns of use of drugs for premedication. Methods: It is a cross-sectional descriptive study performed in the department of Anesthesiology at KIST Medical College and Teaching Hospital from 15th Feb 2022 till 14th Feb 2023, after obtaining ethical clearance from the Institutional Review Committee. The total sample size was 385 which were calculated using Cochrane formula. All the patients who belonged to American Society of Anesthesiologists (ASA) I-III undergoing elective surgical procedures and administered premedication were selected for the study only after their informed written consent. Results: Out of 496 cases, 385 (53.8%) of patients were selected and pre-medications were recorded. The premedication used were metoclopramide 375 (97.4%), pantoprazole 375 (97.4%), paracetamol 52 (13.5%), glycopyrrolate 26 (6.75%), pregabalin 21 (5.45%), alprazolam 20 (5.19%), and lorazepam 1 (0.25%). Types of surgeries categorizations were 59.22 % general surgery, 17.92 % Gynecological and Obstetrics, 17.14 % Orthopedic cases, 4.67 % ENT and 1.04 % dental cases. Conclusion: The most commonly administered pre-anesthetic drugs were metoclopramide, pantoprazole, paracetamol, pregabalin, alprazolam and then lorazepam.

Objective: To compare the effects of Dexmedetomidine and Fentanyl on the blunting of the hemodynamic response to intubation. Study Design: Comparative cross-sectional study. Place and Duration of Study: Combined Military Hospital Peshawar Pakistan, from Jan to Aug 2021. Methodology: This study was conducted on 908 patients who were ASA grades I and II and underwent general anaesthesia for any surgical procedure. Group-A received Dexmedetomidine, while Group-B received Fentanyl and routine preanaesthetic medications. Hemodynamic responses in both groups were recorded and compared in heart rate and mean arterial pressure at baseline, 2, 5, and 10 minutes after the intubation. Results: Out of 908 patients equally divided into two groups, 579 (63.7%) were males, while 329 (36.3%) were females. The mean age of patients included in our study was 33.445 6.363 years. A significant increase in heart rate was found at 2, 5 and 10 minutes in the group who took Fentanyl compared to the group who took Dexmedetomidine (p-value 0.001). ignificant increases in mean arterial pressure were found at 2 and 5 minutes (p-value 0.019 and 0.007, respectively) in the Fentanylgroup, while no significant difference was found at 10 minutes in either group (p-value 0.914). Conclusion: In this study, Dexmedetomidine was found to be a better agent in providing hemodynamic stability during and after the process of intubation compared to Fentanyl.

Background & Objectives: Intra and postoperative nausea and vomiting under regional anesthesia is a common problem in cesarean sections. Globally, the incidence has been documented to be from 40−80%, and various drugs have been used to control it. We aimed to determine if a subhypnotic dose of midazolam will be effective to reduce intraoperative nausea and vomiting during an elective cesarean section under spinal anesthesia. Methodology: A randomized interventional study was done through a period of 6 months from March 11, 2019 to September 11, 2019 and included 100 full term pregnant women undergoing elective cesarean sections (CS) under spinal anesthesia. The participants were allocated to one of two groups: Midazolam group (received midazolam 1 mg bolus then 1.0 mg/h infusion after umbilical cord clamping), and placebo group (received normal saline). Bellville score was used to evaluate nausea and vomiting (N/V). The hemodynamic parameters were monitored at three-minute intervals. Results: No statistically significant differences were noted between study groups regarding mean arterial pressure, heart rate and total ephedrine used in the two groups. Intraoperative nausea and vomiting were recorded midazolam (81.6%) with a significant association between prevention of intraoperative N/V and receiving midazolam. Insignificant association was noticed between the level of sedation and receiving midazolam. Conclusion: Administration of low dose of midazolam during cesarean sections under spinal anesthesia can lessen intraoperative nausea/vomiting without significantly effect on blood pressure or heart rate and without any negative side effects.
