Utilization of ancillary studies in thyroid fine needle aspirates: A synopsis of the National Cancer Institute Thyroid Fine Needle Aspiration State of the Science Conference

Abstract: The National Cancer Institute (NCI) sponsored the NCI Thyroid Fine Needle Aspiration (FNA) State of the Science Conference on October 22-23, 2007 in Bethesda, MD. The 2-day meeting was accompanied by a permanent informational website and several on-line discussion periods between May 1 and December 15, 2007 (http://thyroidfna.cancer.gov). This document summarizes matters regarding the utilization of ancillary studies in thyroid FNA (http://thyroidfna.cancer.gov/pages/info/agenda/).

“…Nodules that demonstrate either isofunction or hypofunction on a radionuclide scan should be further assessed by ultrasound. [6][7][8][9][10] Thyroid nodules initially detected by imaging studies may require investigation by FNA, depending on their size and the type of imaging method used at the time of their initial discovery. For example, thyroid nodules incidentally detected by fluorine F 18 2-fluoro-2-deoxy-D-glucose positron emission tomography ( 18 F-FDG-PET) have a relatively high risk of malignancy, [17][18][19][20] and when uptake is localized focally within the thyroid, FNA should be performed to rule out malignancy.…”

“…[1][2][3][4][5] This review was based largely on the NCI conference and summarized the state of the art for indications to perform a thyroid FNA, pre-FNA requirements, diagnostic terminology, and post-FNA testing and treatment options. Summaries of committee findings for training and credentialing 6 and utilization of ancillary techniques in thyroid FNA 7 can be found elsewhere.…”

“…Therefore, a multidisciplinary conference was held at the National Cancer Institute in Bethesda, Maryland, in October 2007, and the proceedings were published in June 2008. [1][2][3][4][5][6] Ultimately, a uniform, 6-tiered reporting system was proposed with defined diagnostic categories, standardized nomenclature, and predictive risk of malignant outcomes for each diagnostic category. 7,8 Since the BSRTC was introduced, the ''atypia of undetermined significance or follicular lesion of undetermined significance'' (AUS/FLUS) category has received significant attention within the cytology literature, and it has been the most controversial.…”
