Utility of respiratory ward‐based NIV in acidotic hypercapnic respiratory failure

Abstract: NIV could be used on medical wards for patients with pH 7.16 or greater on their first admission, although more conservative values should continue to be used for those with a second or subsequent episodes of AHRF.

“…The inclusion of only patients at first presentation is due to the differences that have been identified previously in the literature and specifically in this cohort. This has including the more rapid progression from ED onto the NIV service of those who are on the second or later presentation, which presumably occurs due to their pre-identified risk factor of prior NIV, and proven previous efficacy of NIV treatment [16].…”

“…A recent analysis 130 of UK COPD patients followed after one or more hospital admissions requiring NIV found mortality rates of between 45% and 57% at 1 year, between 62% and 68% at 3 years and between 72% and 75% at 5 years. This was broadly consistent with the 1-year mortality rates in the usual-care arm of one post-hospital study 97 included in this report.…”

“…129 Applying these risks and the all-cause mortality risk reported in Garcia-Aymerich et al (2003), 152 and assuming they remained constant for the cohort's lifetime, the model projected cumulative mortality similar to that reported in long-term (5-year) studies. 86,130 The mortality rates reported in post-hospital studies and cohorts of COPD patients discharged from hospital are reported in Appendix 13 and the baseline survival curves for the stable and post-hospital populations are illustrated in Appendix 15.…”

“…[ 13 ] Dave reviewed outcomes in 498 patients requiring 731 admissions for acute hypercapneic respiratory failure, including 109 patients who required noninvasive ventilation. [ 17 ] Admission glucose levels were similar in patients managed on the ward and patients managed in a high dependency unit or ICU. Our studies did not identify a relationship between LOS, mortality, or re-admission.…”
