Utility of blood procalcitonin concentration in the management of cancer patients with infections

Abstract: Diagnosis of infections in cancer patients is usually problematic since differentiating between infection and fever of unknown origin is often a considerable clinical challenge. In general, increase concentration of blood procalcitonin (PCT) is associated with severe bacterial infection. PCT with an optimal cutoff level of 0.5 ng/mL seems to be the most helpful biochemical parameter in detecting severe infections, mainly bloodstream infection, in patients with hematological cancers. In all clinical situations,… Show more

“…To date, more than 170 different biomarkers have been evaluated for use in sepsis complications and outcome prediction, evaluation of therapy response, and development of organ dysfunction [ 95 ]. Despite this number, only a few of them, including C-reactive protein (CRP) [ 96 ] and procalcitonin (PCT) [ 97 , 98 ], are routinely employed in clinical practice due to low specificity or sensitivity of the proposed biomarkers [ 95 , 99 ]. Even CRP and PCT therapeutic values have been challenged due to difficulties in distinguishing sepsis from other inflammatory diseases [ 98 , 100 , 101 ].…”

“…This theory is based on the observations that some classes of molecules, along with tissue damage and the resulting inflammation, increase cell proliferation. Certainly, the promotion of cancer risk is due not only to cell proliferation but also due to prolonged cell proliferation in an environment containing inflammatory cells, cytokines, activated stroma, and agents promoting DNA damage [ 65 , 66 ].…”

“…In patients with cancer, a diagnosis of infection is usually problematic. The clinical challenge is to differentiate infection and fever of unknown importance [21]. Early diagnosis and stratification of sepsis risk increases the chances of initiating well-timed treatment, which improves the prognosis of patients [22].…”
