Objectif : Évaluer l'évolution de la fonction rénale, après dérivation urinaire transiléale selon Bricker, chez les patients présentant des troubles vésicosphinctériens d'origine neurologique.Matériels et méthodes : Entre août 2004 et décembre 2007, une étude prospective évaluant la fonction rénale, après dérivation transiléale non continente (UCTI) selon Bricker, a été menée sur 27 patients. Ils avaient une mesure de la clairance de la créatinine sur 24 heures et une urotomodensitométrie, avant la chirurgie, à trois mois postopératoire, puis de manière annuelle. Les pathologies neurologiques étaient une sclérose en plaques (SEP) chez 22 patients, une paraplégie chez deux patients, une myélite transverse chez un patient, un spina bifida avec myélomé-ningocèle chez un patient et une neuropathie périphérique secondaire à un Gougerot-Sjögren chez une patiente.Résultats : Le recul moyen était de 29,8 AE 11,2 mois. Il n'existait pas de différence significative de la valeur de la clairance de la créatinine mesurée sur 24 heures (78,8 AE 36,8 ml/min en préopératoire, contre 82,4 AE 36,1 ml/min en postopératoire, p = 0,7 [test exact de Fisher]). Deux patients (7,4 %) ont présenté une sténose urétérale. Aucune sténose stomiale n'a été notée. Aucune lithiase du haut appareil n'a été retrouvée.Conclusion : À moyen terme, chez les patients neurologiques ayant une UCTI, il n'existe pas de dégradation de leur fonction rénale. Materials and methods: From August 2004 toDecember 2007, a cohort study was carried out on 27 patients who had laparoscopic cystectomy with ileal conduit diversion. Creatinine clearance and IVP measurements were taken before surgery at 3 months postoperative and then on an annual basis. The neurological diseases were multiple sclerosis in 22 patients, paraplegia in two patients, a transverse myelitis in one patient, spina bifida with myelomeningocele in one patient and peripheral neuropathy secondary to Gougerot-Sjögren in one patient.Results: Mean follow-up was 29.8 AE 11.2 months. No significant difference in the value of creatinine clearance (78.8 AE 36.8 ml/min preoperatively, versus 82.4 AE 36.1 ml/min postoperatively, P = 0.7 [Fisher's exact test]) was noted. Two patients (7.4%) had ureteral stenosis. No stomal stenosis or renal lithiases were reported.Conclusion: In the medium term, in patients with UCTI for neurogenic voiding dysfunction, there is no deterioration of renal function.
