Acta Obstet Gynecol Scand 2001; 80: 511-518 Aim of the study. To investigate intrauterine infection as a cause for unexplained stillbirth.Methods. Chorioamnionitis was studied in a material of stillbirths (117 subjects from the years 1985-1994) from a region in the south Sweden. Control material (126 alive and healthy newborns and with healthy mothers) was gathered from the same region.Results. Chorioamnionitis was a common diagnosis both with stillbirths and 'healthy' deliveries (82 and 68%, respectively). Extension of the inflammation to decidua basalis was seven times more common among stillbirths than among controls (odds ratio 7.2, confidence interval 2.8-21.9). The most common bacteria found at cultures were Escherichia coli, Coagulase negative staphylococcus, Enterococcus faecalis and group B Streptococcus. The risk for stillbirth was doubled if both inflammation and bacteria were present (odds ratio 2.3, confidence interval 0.92-5.8). Meconium discharge was more common among stillbirths than controls (odds ratioΩ4.7, confidence interval 1.7-14). There were no differences in any respect regarding macerated and non-macerated stillbirths. Our findings are similar to the results from studies in developing countries except for the higher incidence of stillbirths in such countries.Conclusions. Thus, a large part of otherwise unexplained stillbirths might be due to ascending infections.

Maternal tolerance of the fetal hemiallograft suggests that immunomodulation occurs during gestation. Therefore, recurrent spontaneous abortion (RSA) may represent a failure of the immune changes that maintain pregnancy. We hypothesized that fertile women but not women with RSA may lose their immune responses to recall antigens when pregnant. This phenomenon has been seen in immunosuppressed transplant recipients and is associated with graft survival. Therefore, we evaluated proliferative responses to recall antigens in four groups of women: group 1, nonpregnant fertile women with no history of pregnancy loss and at least one prior healthy pregnancy, n ϭ 13; group 2, nonpregnant women with a history of three or more spontaneous abortions, n ϭ 28; group 3, healthy pregnant women between 6 and 9 wk of gestation without a history of prior pregnancy loss, n ϭ 15; and group 4, pregnant women between 6 and 9 wk of gestation, with a history of RSA, n ϭ 22. Proliferative responses of peripheral blood leukocytes to the recall antigens influenza and tetanus, alloantigens, and phytohemagglutinin were determined prospectively. Positive responses (stimulation index Ͼ 3) to recall antigens (a response to either influenza or tetanus was considered positive) were as follows: group 1 (nonpregnant fertile women), 11/13 (85%); group 2 (nonpregnant RSA women), 24 / 28 (86%); group 3 (pregnant fertile women), 4/15 (27%) ( P Յ 0.007); and group 4 (pregnant RSA women), 13/22 (59%) ( P ϭ NS). In group 4, there was 100% fetal survival in the nine women who lost responsiveness to recall antigens; however, in the 13/22 patients who responded to recall antigens, 9/13 (69%) had a repeat spontaneous abortion. These findings suggest that immunosuppression, indirectly measured by proliferation to recall antigens, is necessary for early pregnancy maintenance. Furthermore, this approach may be useful for predicting pregnancy outcome for women with RSA and may provide a useful means for designing and monitoring therapies. ( J. Clin. ÖzetPerinatal beyin hasarı ve izleyen dönemlerde serebral palsi görülmesi intrauterin enfeksiyonlar ve inflamatuvar cevap ile ilişkilidir. Prematürelik ve membranların erken açılması ise enfeksiyonlar ve inflamasyon ile yakından ilişkilidir. Hem enfeksiyonun hem de inflamasyonun prematüre doğum ile birlikte bulunması ileride oluşabilecek serabral palsi riskini artırmaktadır ve yenidoğanlarda kraniyal görüntülemede sıkça görülen beyaz madde hasarların-dan birisi olan periventriküler lökomalazi serebral palsi için major risk faktörüdür. İntrauterin enfeksiyon sürecinde salınan inflamatuvar sitokinler beyinde beyaz madde hasarı oluşmasında önemli rol oynar. Maternal intrauterin enfeksiyonların, intraventriküler kanama, neonatal beyaz madde hasarı ve serebral palsi ile sonuçlanan preterm eylem riskini artırdığı birçok çalışma ile gösterilmiştir. Proinflamatuvar sitokinler olan interlökin-1,interlökin-6 ve tü-mör nekroz faktör-α perinatal intrauterin enfeksiyonların neonatal beyin hasarına yol açmasına aracılık edebilir ve bu profinflamatuvar sitokin kaskadı-nın ikinci trimesterin sonuna yakın dönemde durdurulması ileride oluşabilecek nörogelişimsel bozuklukları önleyebilir. Anahtar Kelimelerİntrauterin Enfeksiyon; Sitokinler; Perinatal Beyin Hasarı Abstract Perinatal brain injuries and the subsequent development of cerebral palsy are closely associated with intrauterine infections and inflammatory response. Premature prenatal rupture of membranes and premature births are also closely linked to infections and inflammation, and the presence of both infection / inflammation and premature birth together greatly increase the risk for cerebral palsy. Periventricular leukolamacia, a common neonatal brain white matter lesion, is a major risk factor for cerebral palsy. Inflammatory cytokines released during the course of intrauterine infection play an important role in the genesis of brain white matter lesion. Maternal intrauterine infection appears to increase the risk of preterm delivery, which in turn is associated with an increased risk of intraventricular hemorrhage, neonatal white matter damage, and subsequent cerebral palsy. Proinflammatory cytokines IL-1, IL-6 and Tumor necrosis factor-α might be the link between prenatal intrauterine infection and neonatal brain damage, and interrupting the proinflammatory cytokine cascade might prevent later disability in those born near the end of the second trimester.
