Uterine rupture and labour after a previous low transverse caesarean section

Abstract: This study reviewed all cases of complete uterine rupture (UR) in pregnancy during the decade 1993-2002. In 69 412 deliveries, 5320 women had a single prior caesarean section. Of these, 4021 had a trial of labour and 3129 (77.8%) delivered vaginally. In nine (0.22%) cases, the previous transverse scar ruptured during labour. None of these nine ruptures resulted in maternal or fetal death, peripartum hysterectomy or fetal morbidity. In our practice, a trial of labour in women with a previous low transverse caes… Show more

“…In our previous investigation, we observed a 0.5% rate of uterine rupture in women with doublelayer closure, which was similar to that in other cohort studies in which it was standard practice. 4,5 Our overall rate of 1.2% was elevated because of the high proportion of women who had previous single-layer closure, which was associated with 3.1% of uterine ruptures. This rate is similar to the rate of uterine rupture (2.9%, 15 cases over 517 trials of labor) reported by Rozenberg et al 6 in their prospective cohort study in France.…”
