Uterine-preserving pelvic organ prolapse surgery using the UPHOLD LITE vaginal support system

Abstract: This article aims to evaluate the safety and outcome of women with pelvic organ prolapse (POP) treated by a minimally invasive bilateral sacrospinous hysteropexy (UPHOLD LITE Vaginal Support System, Boston Scientific) without concomittent anti-incontinence surgery. This retrospective study was conducted between 2014 and 2016. Evaluated items included surgical parameter and postoperative outcome. Three hundred thirteen women with POP were eligible and 22 were excluded because of history of… Show more

“…None of the women with mesh extrusion underwent concurrent hysterectomy. The mesh extrusion rate was comparable to previous studies using the Uphold system for POP surgeries, ranging from 0 to 6.6% 6,10,[15][16][17][18][19][20][21][22] . There was no mesh extrusion after the 52nd surgery, which may indicate that there was a learning curve for the Uphold mesh surgery.…”

“…In the present study, 63% of the women had previously undergone surgery in the anterior compartment, and most patients had undergone an isolated Uphold procedure. Other studies have demonstrated low re‐operation rates of 0%–3% about 2 years after Uphold procedure 18–20 . This might reflect the shorter follow‐up period or a different access to reoperation.…”

“…Therefore, the findings of the current study cannot expand to other populations, such as surgical treatment for patients with gynecological malignancy and/or pelvic floor dysfunction-related benign diseases. However, it is well-known that patients with gynecological cancer may be managed by much complicated surgery, and patients treated with pelvic organ prolapse surgery may have a higher risk of existed or subsequent lower urinary tract symptoms (Bisch et al, 2021; Chang et al, 2019; Chang et al, 2020; Liu et al, 2019), in which all may add the uncertainty to test our hypothesis. The enrolled patients in the current study were limited to benign gynecological tumors which needed surgical excision, regardless of use or no-use of laparoscopy assistance.…”
