Background: Liposarcoma of the broad ligament is extremely rare, and there have been no reports of atypical lipomatous tumor/well-differentiated liposarcoma (ALT/WL) in this site. Case: A 57-year-old woman presented with an endometrial cancer concomitant with a voluminous parauterine mass. Histopathologic examination showed an atypical lipomatous tumor without carcinomatous elements. Therefore, the diagnosis of malignant mixed Mullerian tumor with liposarcomatous differentiation was ruled out. Results: At explorative laparotomy, a tumor of 100 · 80 · 35 mm arising from the broad ligament was inspected. Total abdominal hysterectomy, with bilateral salpingo-oophorectomy and full pelvic lymphadenectomy, without para-aortic lymphadenectomy, were performed. There are no previous reports of atypical lipomatous tumor of the broad ligament. Therefore, the risk of dedifferentiation/metastasis is unknown. Complete surgical excision with tumor-free margins was achieved, and, therefore, no adjuvant chemo/radiotherapy was administered. Conclusions: This report illustrates that ALT/WL could occur, separately from the uterus, in the uterine ligaments and should thus be treated as a softtissue liposarcoma. ( J GYNECOL SURG 29:145)
