Uterine artery embolisation for symptomatic fibroids: the effect of the large uterus on outcome

Abstract: Objective The aim of this study was to evaluate the efficacy of uterine artery embolisation (UAE) in myomatous uteri larger than 24 week's gestation (780 cm 3 ). Design Prospective case control study.Setting Universitas Hospital, University of the Free State, Bloemfontein, South Africa.Population Sixty-one women, who underwent UAE, were included in the study. The study group comprised of 12 women with uteri !780 cm 3 and the control group 49 women with uteri <780 cm 3 . Methods UAE was performed and the differ… Show more

“…16 Recent evidence has supported the efficacy of UAE in patients with massive uterine fibroids. 17 VTE is a very rarely reported complication of UAE affecting <0.1%, compared with a case prevalence of 1 -2% for hysterectomy. 13,18 In a recent study several surrogate markers of hypercoagulability were noted to increase post-UAE, suggesting that the procedure may induce a prothrombotic state.…”

“…Incorporation of UAE into routine clinical practice is widely endorsed and its role in heavy menstrual bleeding has been investigated recently. [13] Previously, a local study by Prollius et al [10] reported a mean reduction in uterine volume of 188 cm 3 in 51 women on follow-up at 12 months. There are limited local data on the long-term effect of UAE on symptomatic fibroid volume reduction using follow-up ultrasound imaging.…”

“…A growing body of evidence supports the role of UAE in the treatment of symptomatic fibroids since its introduction by Ravina et al in 1995. [7][8][9][10][11][12] Since uterine fibroids receive their blood supply via end arteries and do not depend on collateral arteries, occlusion causes ischaemic necrosis and shrinkage (Fig. 2).…”

“…6 This treatment has been shown to control both menorrhagia and symptoms caused by the bulk of fibroids in 85-90% of patients. 7,8 Complications are rare and include post-treatment sepsis, late infection of the endometrium or myometrium, severe ischemic injury to the uterus (1-2%), spontaneous passage of fibroid tissue and amenorrhea. 7,8 Initially, polyvinyl alcohol particles were used as the embolic agent, injected through catheters placed in the uterine arteries.…”
