Regional anaesthesia, referred to as regional blocks, is one of the most frequently used methods of anaesthesia for surgery and for pain management. Local anaesthetic drug should be administered as close to the nerve as possible. If administered too far away, this may result in insufficient block. If it is administrated too close, severe nerve damage can occur. Neurostimulation techniques and ultrasound imaging have improved the effectiveness and safety of blockade, but the risk of nerve injury with permanent nerve disfunction has not been eliminated. Intraneural administration of a local anaesthetic damages the nerve mechanically by the needle and the high pressure generated by the drug inside the nerve. In many studies, injection pressure is described as significantly higher for unintended intraneural injections than for perineural ones. In recent years, the concept of combining techniques (neurostimulation + USG imaging + injection pressure monitoring) has emerged as a method increasing safety and efficiency in regional anaesthesia. This study focuses on the contribution of nerve identification methods to improve the safety of peripheral nerve blocks by reducing the risk of neural damage.

Regional anaesthetic techniques are an essential element in acute and postoperative pain management [1]. Perineural deposition of a local anaesthetic leads to reversible blockade of sodium channels and temporary inhibition of nociceptive conduction. Nerve conduction blockade is based on the all-ornone law; hence, regional anaesthesia is the most efficacious form of analgesia. The benefits of regional analgesic techniques are particularly noticeable in trauma-orthopaedic, large-joint reconstructive and thoracic surgical procedures (Table 1) [2]. Depending on the local anaesthetic (LA) used, the duration of analgesia is limited to a maximum of 12-16 hours. For most scheduled procedures (usually performed between 8 a.m. and 3 p.m.), the blocking effect ends in the late evening or night [3, 4]. In most departments, compared to daytime, night hours are associated with a smaller staffing of medical personnel who can respond quickly to pain reported by the patient, which creates a risk of lack of optimal pain control. The imperative in the treatment of postoperative pain is to provide the patient with optimal analgesia and comfort, especially during the period of the most severe complaints, i.e. on the first postoperative day, including night hours. Hence, many techniques are used in clinical practice to prolong standard analgesia after perineural deposition of LA. The researchers of the present study aimed to explore the effects and mechanism of general control of amino acid synthesis 5 like-1 (GCN5L1) in the anaesthesia-induced nerve injury model. In brief, the mRNA and protein of GCN5L1 expression was up-regulated in the anaesthesia-induced nerve injury model. GCN5L1 protein was increased in anaesthesia-induced nerve injury by inducing oxidative stress. Meanwhile, over-expression of GCN5L1 promoted oxidative stress in vitro. Down-regulation of GCN5L1 inhibited oxidative stress in nerve cells. GCN5L1 suppressed the protein expression of forkhead box O1 (FoxO1) in vivo or in vitro. The activation of FoxO1 attenuated the effects of GCN5L1 on oxidative stress in vitro. Finally, FoxO1 protein attenuated the effects of GCN5L protein on induced nerve injury via suppression of nerve apoptosis and oxidative stress in vivo. The researchers concluded that GCN5L1 regulated anaesthesia-induced nerve injury in newborns through FoxO1 via oxidation effects.
