Background Rett Syndrome (RTT) is a rare, severe, and progressive developmental disorder with intellectual disability. Anesthesia in RTT patients presents a range of challenges. We report a child with RTT who received dental treatment under muscle relaxant-free general anesthesia in our ambulatory center. Case presentation A 15-year-old girl with RTT was admitted to our dental clinic with multiple dental caries and residual roots. Dental treatment was scheduled under ambulatory general anesthesia. After anesthesia induction, a nasal tube was initiated under the guidance of a fiberoptic bronchoscope. Multimodal analgesia, body temperature monitoring, and postoperative nausea and vomiting prevention were applied. No muscle relaxants were used throughout the process. The endotracheal tube was successfully removed after the operation and the patient was discharged home the same day. Conclusion An individualized anesthesia strategy enabled a quick and safe recovery for this RTT patient after dental treatment under muscle relaxant-free general anesthesia.

Antecedentes. La vía aérea es aquella en la que un anestesiólogo entrenado, experimenta dificultad para la ventilación o dificultad para la intubación o ambas. Objetivo. Determinar el desenlace de los pacientes mayores de 18 años con vía aérea difícil y las características de la capacidad de respuesta institucional, en el Hospital Escuela Universitario, Tegucigalpa, Honduras. 2018-2020. Métodos. Estudio Descriptivo Transversal, con una muestra de 80 pacientes con vía aérea difícil. Muestreo no probabilístico por conveniencia. Los datos se ingresaron en EPI-INFO. Se realizó análisis descriptivo como medidas de tendencia central y agrupación. La proporción se estimó utilizando la fórmula: P= Número de personas con vía aérea difícil / número de pacientes mayores de 18 años multiplicado por 100. El protocolo fue aprobado por el Comité de Ética en Investigación Biomédica/FCM/UNAH. Resultados. El 28,8 % tuvieron riesgo anestésico ASA II, el 27,6 % tiene un ASA IIE. El 68,5 % tuvieran Escala Cormack Lehane Grado III, el 7,5 % Grado II, el 23,7 % Grado IV, el 97,5 % de la intubación fue exitosa, 2,5 % fue fallida. El 36,3 % con vía aérea difícil tienen sobrepeso, el 25 % son Obesos tipo I, 13.8 % obesos tipo III y el 8,3 %. Se tuvo el 97,2 % de intubación exitosa y se requirieron en la mayoría hasta 2 intentos para poder lograrlo. Conclusiones. La obesidad constituye un factor de riesgo para ventilación e intubación difíciles, así mismo un riesgo anestésico ASA II y III es también un factor de riesgo para vía aérea difícil.
