Ventral pallidum (VP) is a well-established locus for the reinforcing effects of drugs of abuse and reinstatement of drug seeking. However, VP neurons are at the origin of multiple output pathways, with strong projections to ventral tegmental area (VTA), subthalamic nucleus (STN), lateral hypothalamus, among others, and the roles of these VP output pathways in reinstatement of drug seeking remain poorly understood. Here we addressed these issues using a combination of neuroanatomical tracing and chemogenetic approaches. First, using dual-retrograde tracing, we show that VP neurons projecting to either VTA or STN are recruited during context-induced reinstatement of extinguished alcohol seeking in rats. Then, using chemogenetics, we show modulation of context-induced reinstatement and reacquisition of alcohol seeking via designer receptors exclusively activated by designer drugs excitation or inhibition of the VP. To determine the causal roles of VP ¡ VTA and VP ¡ STN pathways in context-induced reinstatement and reacquisition we used a chemogenetic disconnection approach and show that silencing either the VP ¡ VTA or VP ¡ STN pathways is sufficient to reduce both reinstatement and reacquisition of alcohol seeking. Moreover, these disconnections also each reduced responding and motivation during a progressive ratio test but had no effect on locomotor activity. Together, these results show that multiple ventral pallidal output pathways contribute to relapse to alcohol seeking.

Deep brain stimulation (DBS), the practice of placing electrodes deep into the brain to stimulate subcortical structures with electrical current, has been increasing as a neurosurgical procedure over the past 15 years. Originally a treatment for essential tremor, DBS is now used and under investigation across a wide spectrum of neurological and psychiatric disorders. In addition to applying electrical stimulation for clinical symptomatic relief, the electrodes implanted can also be used to record local electrical activity in the brain, making DBS a useful research tool. Human single-neuron recordings and local field potentials are now often recorded intraoperatively as electrodes are implanted. Thus, the increasing scope of DBS clinical applications is being matched by an increase in investigational use, leading to a rapidly evolving understanding of cortical and subcortical neurocircuitry. In this review, the authors discuss recent innovations in the clinical use of DBS, both in approved indications as well as in indications under investigation. Deep brain stimulation as an investigational tool is also reviewed, paying special attention to evolving models of basal ganglia and cortical function in health and disease. Finally, the authors look to the future across several indications, highlighting gaps in knowledge and possible future directions of DBS treatment.

Хроническое потребление алкоголя снижает уровень циркулирующего в крови ИФР-1, в основном из-за снижения скорости синте-за и секреции его печенью [8,13]. Cнижение содержания ИФР-1 в системном кровотоке может вызывать атрофиче-ский процесс, который наблюдается при ХАМ [14,15]. Не исключено, что потеря мышечных ядер обусловлена раз-витием некомпенсированных апоптотических процессов, признаки которых были ранее обнаружены у пациентов с ХАМ [16]. В проведенном исследовании впервые получено уменьшение количества сателлитных клеток у больных с признаками атрофии МВ, что говорит об истощении их ре-генераторного потенциала.Снижение синтеза белка является ранним признаком поражения скелетных мышц у пациентов с хронической ал-когольной интоксикацией, вероятно, опережающим мор-фологические проявления ХАМ. В дальнейшем возможно использование определения уровня ИФР-1 в плазме крови для ранней скриннинговой диагностики ХАМ.Необходимо продолжить исследования механизмов развития ХАМ с целью разработки патогенетически обос-нованных методов лечения данного проявления алко-гольной болезни. Болезнь Паркинсона (БП) -распространенное ней-родегенеративное заболевание, при котором наиболее эф-фективны дофаминергические препараты. По мере про-грессирования заболевания пациенты с БП, у которых раз-вивается снижение эффективности контроля двигательных симптомов при заместительной дофаминергической тера-пии, начинают принимать значительно большие дозы лево-допы и агонистов дофаминовых рецепторов (АДАР), чем это необходимо [1]. Известно, что при приеме дофаминер-гических препаратов улучшаются память и мышление. На-личие когнитивных и поведенческих расстройств часто спо-собствует увеличению дозы препаратов [2, 3], что приводит к появлению зависимости от леводопы и развитию деструк-тивных поведенческих проблем [4].Дофаминовый дизрегуляционный синдром (ДДС) часто описывают как ятрогенное расстройство в результате дли-тельной симптоматической терапии БП, он выявляется примерно в 3-4% случаев при БП [1].Известно, что компульсивный (навязчивый) прием до-фаминергических препаратов наиболее часто встречается у мужчин с ранним дебютом БП. Алкоголизм, употребление наркотиков, наличие аффективных расстройств в анамнезе -факторы риска развития ДДС [2, 3]. В ранних стадиях БП па-циенты отмечают, что повышение дозы препаратов приводит к увеличению энергии, силы и продуктивности [5]. На фоне приема высоких доз дофаминергических препаратов появля-ются эйфория в период «включения» и дисфория в период «выключения». Больные с нетерпением ждут приема следую-щей дозы и обосновывают необходимость увеличения частоты приема лекарств, несмотря на хорошую двигательную актив-ность и появление дофаминовых дискинезий в период «вклю-чения».
