Using Patient Data to Electronically Identify Palliative Care Patients (S765)

Abstract: rehabilitation (SAR). Hospital administrators are currently working to design programs to decrease hospital re-admittance rates for chronically ill patients. The purpose of this study is to describe the characteristics of patients who received a palliative care consultation (PCC) at one SAR and describe the outcomes of these consults. Research Objectives. To evaluate clinical outcomes of patients who receive palliative care consultation while residing in a sub-acute rehabilitation (SAR) facility.
