Using near-infrared reflectance spectroscopy (NIRS) to assess distal-limb perfusion on venoarterial (V-A) extracorporeal membrane oxygenation (ECMO) patients with femoral cannulation

Abstract

Advancements in NIRS technology seem to have improved its accuracy for continuous, non-invasive monitoring of regional tissue and may provide clinicians with an additional metric to protect the distal portion of the cannulated leg.

“…REBOA施行時には，動脈損傷，シースやバルーンによる血栓塞栓症，遮断末梢側の臓器不全などの重篤な合併症が起こりうる。Manzano–Nunezらの系統的レビューによると，REBOA挿入患者の5.6%にシース挿入部位である鼠径部の合併症（仮性動脈瘤の発生など）が発生し，挿入患者の2.1%に末梢側の下肢切断を要したという 11)。遮断部位以下の虚血予防については，速やかな止血・遮断時間の最小化に加え，前述したpartial REBOAやintermittent REBOAが有用な可能性がある。partial REBOAを安全に行うためには，留置部位における完全遮断時の最小限のバルーン容量を把握しておくとよい。本症例では，透視室で大動脈の完全遮断が15mLで得られることを確認し，循環動態に合わせpartial REBOAを適宜調整することができた。さらに，大腿カニュレーションによるVA–ECMO（veno–arterial extracorporeal membrane oxygenation）導入時，近赤外線分光法（NIRS: near–infrared spectroscopy）を用いて下肢の混合血酸素飽和度をモニターする試みが報告されており 12)，REBOA使用時の虚血モニタリングに有用である可能性がある。また，当院では手技に起因する合併症を減らすべく，REBOA挿入マニュアルを作成している。初心者は血管内治療の熟練者の指導のもとで挿入を行い，指導者の許可を得られた者のみが単独施行できる，とする教育体制をとることで手技の質を担保している。…”
