Background Inflammation is a key component of both acute kidney injury (AKI) and response to cardiopulmonary bypass. Since AKI poses risks to children after cardiac surgery, we investigated the value of inflammatory biomarkers interleukin-8 (IL-8) and tumor necrosis factor alpha (TNF) for predicting AKI and other complications. Methods We enrolled 412 children between 1 month and 18 years old undergoing cardiopulmonary bypass for cardiac surgery. We collected blood both preoperatively and postoperatively (within 6 hours post-surgery) and measured plasma IL-8 and TNF. Results IL-8 and TNF did not predict AKI in children <2 years, but strongly associated with AKI in children ≥2 years. There were significant associations between biomarker levels and age (<2 or ≥2). In children ≥2, patients in the highest tertile of preoperative IL-8 and postoperative TNF had 4.9 [95% CI: 1.8–13.2]- and 3.3 [95% CI: 1.2–9.0]-fold higher odds of AKI compared to those in the lowest tertile. Children <2 with higher biomarker levels also had higher odds of AKI, but the difference was not significant. We also found that postoperative TNF levels were significantly higher in patients with longer hospital stays, and that both postoperative IL-8 and TNF levels were significantly higher in patients with longer ventilation lengths. However there was no evidence that biomarker levels mediated the association between AKI and length of ventilation; they appear to be independent predictors. Conclusions Preoperative IL-8 and postoperative TNF are significantly associated with higher odds of AKI and greater lengths of hospital stays and ventilator use in children ≥2.

Kidney disease represents a serious global health problem. One of the main concerns is its late diagnosis, only feasible in a progressed disease state. The lack of a clinical manifestation in the early stages and the fact that the commonly measured parameters of renal function are markedly reduced only during advanced stages of the disease are the main cause. Changes at the molecular level of the kidney tissue occur even before nitrogenous substances, such as creatinine and urea, start to accumulate in the blood. Renal proximal tubules contain a large number of mitochondria and are critical for the energy-demanding process of reabsorption of water and solutes. Mitochondria are the largest producers of oxygen radicals, which, in turn, increase the susceptibility of kidneys to oxidative stress-induced damage. Free radicals and prooxidants produced during acute or chronic kidney injury may further aggravate the course of the disease and play a role in the pathogenesis of subsequent complications. Prevention might be the solution in CKD, but patients are often reluctant to undergo preventive examinations. Noninvasive markers and the possibility to obtain samples at home might help to increase compliance. This review will provide an overview of the possible uses of markers of oxidative status in noninvasive biofluids in patients with renal disease.

Objectives: Scant information is available on factors for predicting the rate of decline in kidney allograft function beyond 1 year posttransplant. We invest igated whether urinary enzymes (alanine amino transferase, alkaline phosphatase, aspartate aminotransferase, N-acetyl-β-D-hexosaminidase, and γ-glutamyl transpeptidase) in the late postoperative period can predict the decline in estimated glomerular filtration rate. confidence interval, 1.10-3.83; P = .023) over 2 years. It also predicted the drop in estimated glomerular filtration rate ≥ 25% after 1 year (odds ratio, 2.62; 95% confidence interval, 1.07-6.37; P = .034) and 2 years (odds ratio, 2.75; 95% confidence interval, 1.12-6.73; P = .027). Combined with time after transplant, urinary aspartate aminotransferase had good power for predicting an estimated glomerular filtration rate decrease ≥ 25% after 2 years of follow-up. Conclusions: Higher urinary activity of aspartate aminotransferase in the late posttransplant period is useful for identifying transplant patients who are at risk for progressive loss of graft function.
