High-fidelity simulation (HFS) is an educational tool commonly used by professional education programs of medicine and nursing for student training and assessment. Although its use in occupational therapy (OT) education is emerging, implementation of simulation in OT education varies broadly across programs, and, in general, the tool remains underused. This paper describes how the OT education program at Samuel Merritt University (SMU) has systematically expanded the use of HFS in various formats throughout the curriculum. The specific processes and procedures of using HFS in facilitating student critical thinking and clinical skills are explained to emphasize the potential educational value of HFS. Although student responses to simulation have been positive, methodical studies are needed to validate the use of this teaching and learning tool in OT education. The paper also suggests additional ideas for implementing aspects of HFS based on various levels of resources available to an OT education program.

Absztrakt: Az egészségügyi, orvosi szimulációs oktatás alapja, hogy klinikai szituációkat, gyakorlati feladatokat modellez szimulátorok, számítástechnikai megoldások, esetleg humán szereplők segítségével. Így az oktatott a legvalósághűbb környezetben sajátíthatja el a készségeket. A közleményben bemutatjuk a szimulációs oktatás múltját, technológiai hátterét, valamint a szimulációs oktatás jelenlegi helyzetét Magyarországon, melynek megkérdőjelezhetetlen szerepe van az egészségügyi felsőoktatásban. Számos pozitív hatása van hallgatók és oktatók számára, továbbá az intézetek számára is számtalan előnyt tartogat. Az egységek feladata, hogy hidat teremtsen az elméleti és a gyakorlati képzés között, felkészítve a hallgatókat a valódi orvosegészségügyi életpálya kihívásaira. Bár a terület több évszázada ismert módszertan az orvos- és egészségtudományi képzésben, csak a XXI. században indult jelentős fejlődésnek a technológia robbanásszerű előrehaladásának köszönhetően. Az innovatív megoldások révén egyre élethűbben lehet modellezni bonyolultabb orvosi feladatokat is. Már hazánkban is használják az oktatás során a 3D nyomtatott segédeszközöket, virtuális- és kiterjesztettvalóság-megoldásokat. A magyarországi „Skill Labor Hálózat” fejlesztése mellett a szimulációs oktatás hazai módszertanának kialakítása is megkezdődött, ennek sikerességét jelzi több, oktatást segítő kézikönyv megjelenése is. A hálózat négy egyetemi skillközpontot és 16 oktató kórházat foglal magában. A skill-laborok eddig hiányzó láncszemek voltak az elméleti és a klinikai oktatás között, melyekben magas szakmai színvonalú gyakorlati készségeket sajátíthatnak el a hallgatók mind a graduális, mind a posztgraduális képzés során. A kezdeti fejlesztések sikeresnek tekinthetők, ezt tükrözi a nagy érdeklődés, a résztvevők száma és a hallgatók pozitív visszajelzései is. Közleményünk átfogó jelleggel betekintést ad a magyarországi egészségügyi szimulációs oktatás jelenlegi helyzetébe, illetve felveti a potenciális fejlesztési irányokat, gyakorlati példákkal szemléltetve. Simulation based learning in nursing education provides learners with opportunities to practice real-life experiences. Enhancing the education of nurse practitioners (NPs) with simulation based teaching and learning strategies has not been well investigated. There is limited evidence related to learning outcomes and the use of high fidelity simulation or standardized patients. In an Ontario Primary Health Care Nurse Practitioner (PHCNP) Program, the use of a multi-model simulation learning activity was piloted with a group of NP learners. The learning activity consisted of three scenarios, each representing typical conditions seen in primary health care across the lifespan. Each scenario was carefully developed with consideration of curriculum goals, use of simulation technology or standardized patients, and the role of faculty facilitators. Learners worked in pairs as a team to complete a focused history and physical examination, formulate a diagnosis, and develop a plan of care or action for the patients. Following each of the three scenarios, the learner teams received focused feedback on their performance. A guided group reflection was conducted following the learning activity. The feedback from the learners was positive, with a recommendation to include similar learning opportunities earlier in the NP curriculum. The learners valued the active learning process, including peer collaboration and group debriefing. Although the findings from this pilot included a small group of learners, there are valuable considerations for nursing faculty teaching in NP programs with a primary health care focus.
