Using Benefit-Cost Ratio to Select Universal Newborn Hearing Screening Test Criteria

Abstract: Objectives Current protocols presumably use criteria that are chosen on the basis of the sensitivity and specificity rates they produce. Such an approach emphasizes test performance, but does not include societal implications of the benefit of early identification. The purpose of the present analysis was to evaluate an approach to selecting criteria for use in UNHS programs that utilizes BCR to demonstrate an alternative method to audiologists, administrators, and others involved in UNHS protocol decisions. … Show more

“…The diagnosis was set in the period up to 12 months. The US National Center for Review, Evaluation and Management of Hearing Screening reports that detection and treatment of hearing impairment at birth for just one child saves about $ 400,000 in special education costs 17 . Results from 12,708 newborns report on screening passing of 99%, while 23/12,708 were scheduled for further auditory monitoring 11 .…”

“…With most births being covered by a capitated reimbursement system and no additional reimbursement being provided for NHS, minimizing costs to the birthing facility is essential 20, 5258. The cost of molecular diagnostics have the potential to gradually decrease as robotic PCR and automated sequencing systems are implemented that allow higher throughput of samples with less manpower utilization and some economies of scale.…”

“…Intervention with hearing aids before 3 months of age is beneficial for hearing-impaired children (Vohr et al 2008) and even slight or mild hearing impairments (16 to 30 dB HL) should be detected, as these have adverse consequences for speech and language development (Olusanya & Newton 2007). In a lifetime monetary perspective, early intervention is characterized by high benefit/cost ratio (Porter et al 2009). A prerequisite for the very early intervention is rapid, objective, and valid methods for precise assessment of hearing thresholds, thereby allowing the use of hearing aids with high acoustic gain for soft sounds (cf.…”
