Using a symptom observation chart for dying patients: Review of a service improvement tool.

Abstract: 57 Background: In July 2014, following judicial review, the Liverpool Care Pathway for Dying Patients was withdrawn from the UK. An individualised approach to care for dying patients was advised, laid out in “One chance to get it right”.
