ObjectivesEffective secondary stroke prevention strategies are suboptimally used. Novel development of interventions to enable healthcare professionals and stroke survivors to manage risk factors for stroke recurrence are required. We sought to engage key stakeholders in the design and evaluation of an intervention informed by a learning health system approach, to improve risk factor management and secondary prevention for stroke survivors with multimorbidity.DesignQualitative, including focus groups, semistructured interviews and usability evaluations. Data was audio recorded, transcribed and coded thematically.ParticipantsStroke survivors, carers, health and social care professionals, commissioners, policymakers and researchers.SettingStroke survivors were recruited from the South London Stroke Register; health and social care professionals through South London general practices and King’s College London (KCL) networks; carers, commissioners, policymakers and researchers through KCL networks.Results53 stakeholders in total participated in focus groups, interviews and usability evaluations. Thirty-seven participated in focus groups and interviews, including stroke survivors and carers (n=11), health and social care professionals (n=16), commissioners and policymakers (n=6) and researchers (n=4). Sixteen participated in usability evaluations, including stroke survivors (n=8) and general practitioners (GPs; n=8). Eight themes informed the collaborative design of DOTT (Deciding On Treatments Together), a decision aid integrated with the electronic health record system, to be used in primary care during clinical consultations between the healthcare professional and stroke survivor. DOTT aims to facilitate shared decision-making on personalised treatments leading to improved treatment adherence and risk control. DOTT was found acceptable and usable among stroke survivors and GPs during a series of evaluations.ConclusionsAdopting a user-centred data-driven design approach informed an intervention that is acceptable to users and has the potential to improve patient outcomes. A future feasibility study and subsequent clinical trial will provide evidence of the effectiveness of DOTT in reducing risk of stroke recurrence.

Uuden kotidialyysihoidon sähköisen palvelun tavoitteena on sujuvoittaa ja tukea potilaiden hoitoa. Palvelun loppukäyttäjien huomioiminen ja osallistaminen kehittämistyöhön on tärkeää, jotta palvelu voidaan ottaa laajasti käyttöön ja se koetaan hyödylliseksi. Tässä artikkelissa kuvataan tutkimus, jonka tavoitteena oli selvittää kotidialyysipotilaiden ja heitä hoitavien ammattilaisten tarpeita ja kokemuksia uudelle kehitettävälle sähköiselle ratkaisulle, joka sisältää sovelluksen ja erilaisia mittareita. Tutkimus on osa monialaista ’Terveysteknologia predialyysi- ja kotidialyysihoidossa’ -tutkimushanketta. Tutkimusaineisto kerättiin kahdesta etäjärjestelyin toteutetusta työpajasta, joihin osallistui vuoden 2021 lopussa yhteensä viisi potilasta ja neljä ammattilaista. Työpajojen suunnittelussa hyödynnettiin tutkimusprojektissa aiemmin kerättyä laajaa haastattelu- ja arviointitutkimusaineistoa. Työpaja-aineistot analysoitiin laadullisin menetelmin. Potilailla oli jonkin verran käyttökokemusta uudesta kotidialyysin sähköisestä ratkaisusta. Potilaat näkivät mittareista ja sovelluksesta koostuvassa kokonaisuudessa paljon potentiaalia. He kokivat tärkeänä mahdollisuudet omien mittaustulosten seurantaan ja tietoperusteiseen hoitoon liittyvään päätöksentekoon. Potilaiden mielestä jatkuva mittaaminen ja kyselyiden täyttäminen oli kuitenkin ajoittain raskasta. Lisäksi he kokivat älyrannekkeen käytettävyyden heikoksi. Ammattilaiset olivat tietoisia uudesta sähköisestä ratkaisusta, mutta heillä ei vielä ollut kokemusta sen käytöstä. Ammattilaisten näkökulmasta uuden ratkaisun tulee istua osaksi nykyistä teknistä työskentely-ympäristöä. Ratkaisun potentiaalisina hyötyinä nähtiin erityisesti potilaan kokonaiskuvan hahmottaminen ja potilaiden tilan seuraaminen yhdessä potilaiden kanssa. Ammattilaiset toivoivat käyttöönoton tueksi lisäresursseja ja opastusta. Kehitettävässä kotidialyysin sähköisessä palvelussa nähdään olevan potentiaalia, mutta sen laajempaan käyttöön liittyen on tärkeää suunnitella ja sopia toimintatavat ammattilaisten ja potilaiden välillä. Palvelun jatkokehityksessä tulee kiinnittää huomioita käytettävyyteen ja yhteentoimivuuteen nykyisten sovellusten ja järjestelmien kanssa, jotta uuden palvelun käyttö on sujuva ja luonteva osa hoitoa sekä potilaiden että ammattilaisten näkökulmasta. Tulevaisuudessa sähköisten palveluiden avulla voidaan rohkaista useampia potilaita valitsemaan kotihoito kalliimman ja vähemmän yksilöidyn sairaalahoidon sijaan.

More evidence is needed on technology implementation for remote monitoring and self-management across the various settings relevant to chronic conditions. This paper describes the findings of a survey designed to explore the relevance of socio-demographic factors to attitudes towards connected health technologies in a community of patients. Stroke survivors living in the UK were invited to answer questions about themselves and about their attitudes to a prototype remote monitoring and self-management app developed around their preferences. Eighty (80) responses were received and analysed, with limitations and results presented in full. Socio-demographic factors were not found to be associated with variations in participants’ willingness to use the system and attitudes to data sharing. Individuals’ levels of interest in relevant technology was suggested as a more important determinant of attitudes. These observations run against the grain of most relevant literature to date, and tend to underline the importance of prioritising patient-centred participatory research in efforts to advance connected health technologies.
