Usefulness of the Short IQCODE for Predicting Postoperative Delirium in Elderly Patients Undergoing Hip and Knee Replacement Surgery

Abstract: Background/Objective: The prevalence of postoperative delirium in elderly patients is >30%. The objective of this prospective study was to determine the usefulness of the short form of the Informant Questionnaire on COgnitive Decline in the Elderly (short IQCODE) to predict the occurrence of postoperative delirium after elective hip and knee arthroplasty in the elderly. Methods: Consecutive patients, 60 years and older, who were admitted for elective hip or knee arthroplasty were included. The preoperative cog… Show more

“…After examining the full text of these articles, 100 were excluded; 58 because the performance of the RPM was examined using an ineligible (statistically) measure (see ), 28 because they did not examine the outcome POD, seven because they did not use a cohort design, six because they did not describe at least one RPM, and one because of duplicate publication. Ultimately, 30 articles were included in the systematic review …”

“…Known risk factors include poor preoperative functional status, cognitive impairment, depression, alcoholism, vascular disease, comorbidity, and older age. 4,[8][9][10][11][12][13][14][15][16] Delirium is associated with a poorer postoperative outcome and it prolongs the hospital stay. Furthermore long-term cognitive function of the patients who suffered from postoperative delirium can be impaired with a poor functional status.…”

“…9 Despite the strength of the IQCODE, only a few studies have dealt with its usefulness for predicting postoperative delirium. 13,14 The results of this pilot study have prompted us to further investigate and compare the usefulness of the IQCODE and other cognitive screening tests for predicting postoperative delirium in a larger sample size. Additionally, the development and validation of a new informantbased questionnaire that is suitable for use as a screening test for postoperative delirium is being considered.…”
