Usefulness of the 2005 International Society of Urologic Pathology Gleason grading system in prostate biopsy and radical prostatectomy specimens

Abstract: by experienced pathologists in the central pathology department of our hospital, and retrospectively according to IGS by one uropathologist at the central pathology department of another hospital. All patients had RP with no neoadjuvant or adjuvant therapy. We analysed associations of CGS and IGS with biochemical recurrence-free survival (BRFS) after RP. RESULTSThe concordance rates between NB and RP specimens by CGS and IGS were 64.1% and 69.9%. Under-grading and over-grading rates by CGS and IGS were 28.2% a… Show more

“…Regarding the association between the prognostic parameters and biochemical progression in our study, we found a significant association between the conventional parameters including the initial PSa level, gleason score ≥7, and tumor tNM stage with biochemical progression as previously reported by other investigators (37)(38)(39). In addition, early biochemical recurrence was more prevalent in cases with high twist-1, aberrant E-cadherin, and high EZH2 expression.…”

“…However, it will be some time before a sufficient follow-up period has elapsed to do those studies. Both studies by Uemura et al 23 and Billis et al 11 demonstrated that the Gleason scores on needle biopsies using the modified, but not the original, Gleason system correlated with progression after radical prostatectomy. Tsivian et al 31 similarly found that the modified Gleason score, when analyzed in prognostic grade groups (,7, 7, and .7), predicted biochemical recurrence after radical prostatectomy better than did the original Gleason score groups.…”

“…18,23,24 In a study by Uemura et al, 23 the concordance rates for needle biopsies and radical prostatectomy specimens between the original Gleason system and the ISUP modified system were 67% and 70%, respectively. Similarly, Zareba et al 18 showed the biopsyradical prostatectomy did not significantly affect the Gleason score agreement (63.4% and 65.5%, respectively).…”

“…In a similar investigation, Billis and colleagues 10 found that the proportion of cancers with a score of 7 increased from 26% in the classic system to 39% in the ISUP 2005 modified Gleason scoring system; the proportion of cancers with a score of 6 decreased from 68% to 39%. In another study, 11 the ISUP 2005 modified Gleason scoring system resulted in needle biopsy scores that were higher than the scores of the classic system by 16%. In that study, 11 28% of needle biopsy specimens were undergraded or overgraded by the classic system.…”
