Usefulness of Non‐invasive Tests for Diagnosing Helicobacter pylori Infection in Patients Undergoing Dialysis for Chronic Renal Failure

Abstract: The urea breath test seems to be the most reliable diagnostic method for H. pylori infection in patients with chronic renal failure. Serology has a low specificity, and the results of the fecal tests vary widely.

“…Fecal immunoassays perform far more irregularly than other methods do, and indeed the diagnostic accuracy of the different stool tests varies widely [2,3]. One of the reasons for this variation in accuracy is that the antibody composition of the tests that use polyclonal antibodies may change from batch to batch, thus presenting major intratest variability [4][5][6]. Therefore, polyclonal tests are clearly less reliable than are monoclonal tests or urea breath tests [3].…”

“…However, dialyzed patients are often reluctant to undergo invasive procedures such as endoscopies [24] . Because patients with ESRD receive antisecretory drugs and require multiple antibiotic treatments for septic complications, inadvertent eradication of H. pylori infections is possible.…”

“…The UBT seems to be the most reliable diagnostic method for H. pylori infections in patients with chronic renal failure (CRF), and stool antigen tests show heterogeneous results, with substantial differences among manufacturers [24] . However, Wang et al [27] proposed that stool antigen is a noninvasive and reliable tool for screening H. pylori infections before therapy and assessing the success of eradication therapy in patients with ESRD.…”

“…There are various invasive and noninvasive methods for detection of the organism [13]. Some tests are based on enzymatic detection of its strong urease activity [14][15][16]. The urea breath test (UBT) is the most reliable noninvasive test for determining H. pylori status in the general population [16].…”
