Usefulness of Macular Thickness Derived from Spectral-Domain Optical Coherence Tomography in the Detection of Glaucoma Progression

Abstract: Intersession measurements of both cRNFLT and TMT parameters with Cirrus SD-OCT showed excellent reproducibility. TMT parameters using Cirrus SD-OCT may be better than cRNFLT measurements in terms of detecting progressive VF loss. This sensitivity derived from TMT measurements increased when progressive VF loss occurred in the central 10° area.

“…The results of our investigation suggest that the various TR thickness parameters of the RS-3000 OCT are in fact highly reproducible, and therefore may potentially be investigated for glaucoma diagnostic accuracy and accuracy in detection of glaucomatous progression. At the same time our results and the previously published results obtained with another OCT system [ 11 ] suggest that the benefits found in the current study for the TR thickness parameters may represent a general feature, and therefore their evaluation in various OCT systems is proposed.…”

“…Overall, these results confirm the potential diagnostic ability of macular layer thickness that has already been established by numerous studies. 3,7,11,16,[19][20][21][22] These findings also demonstrate the potential of using fellow eyes as internal controls for diagnosing and longitudinally tracking glaucoma progression, avoiding the problem of interanatomic differences and variability. According to Asrani and colleagues, 8,23 macular thickness measurements of the fellow eye can be used as an effective control especially in the absence of a normative macular thickness database, as structural asymmetry has been long established as a typical feature in glaucoma progression.…”

“…25 Macular thinning could also be used as an alternative means of longitudinally tracking glaucoma progression together with pRNFL thickness analysis. 19 Our study has several limitations. Manual segmentation of macular layers, despite adding accuracy to the retinal thickness layer measurements, proves to be a somewhat subjective measure, prone to errors of interpretation.…”
