Usefulness of ED036 kit for measuring serum PIVKA-II levels in small hepatocellular carcinoma

Abstract: As a tumor marker for hepatocellular carcinoma (HCC), serum protein induced by vitamin K absence or antagonist-II (PIVKA-II) has high specificity, yet its sensitivity is relatively low, marking it less suitable to serve as an adjunct in the diagnosis of small HCC. Recently, the ED036 kit (Eisai, Tokyo, Japan), whose detection limit is approximately ten times superior to that of a conventional kit (Eitest MONOP II, Eisai) has been developed. In this study, serum PIVKA-II levels in serum samples from 83 patients… Show more

“…However, Saitoh et al (1994) studied the correlation between AFP and PIVKAII measured by the ABC method in patients with small-sized HCC and found no apparent correlation. Kuromatsu and Okuda also detected no obvious correlation between AFP and PIVKAII measured by the revised EIA kit in their HCC patients (Kuromatsu et al, 1997;Okuda et al, 1999). The sensitivity of GGTII in patients without increased AFP was about 62%, which was less than that in Xu et al's (1985) report (84.6%).…”

“…In the study of Mita et al (1998), the sensitivities of HCC, small-size HCC, and well-differentiated HCC were also much higher than the sensitivities in the study of Saitoh et al (1994) and our study. Okuda et al (1999), Kuromatsu et al (1997), and Saitoh et al (1994) did not report the sensitivity of PIVKAII in the HCC patients without increased AFP, so we are unable to compare the senstivities. However, Saitoh et al (1994) studied the correlation between AFP and PIVKAII measured by the ABC method in patients with small-sized HCC and found no apparent correlation.…”

“…[18][19][20] However, the recently developed high-sensitive assay kit realized sufficiently high sensitivity for clinical use as an equivalent to AFP. 9,21 Possible factors or mechanisms of PIVKA-II increase in the serum of HCC patients are: 1) qualitative or quantitative abnormality of prothrombin precursors; 2) changed activity of ␥-glutamyl carboxylase, i.e., the key enzyme of ␥-carboxylation; 3) lack of vitamin K; and 4) abnormality in the vitamin K cycle. 12 PIVKA-II expression in HCC tissues was confirmed by Nohno, 22 Tamano, 23 and Nakagawa et al, 24 but its expression in small HCC has not yet been studied in depth.…”

“…This opinion should be changed because of the introduction of a high-sensitivity kit (ED036, Eisai, Tokyo, Japan). 9 In the present study, expressions of AFP and PIVKA-II in the tissues of early-stage small HCC were immunohistochemically examined, and their relationships with tumor size, histologic grade, and serum AFP and PIVKA-II levels were investigated.[ …”

“…In small tumors (Ͻ3 cm) sensitivity of AFP levels varied from 26.9% to 39.3% (6,7). In addition, Des-r-carboxyl prothrombin or serum protein induced by vitamin K absence or antagonist-II (PIVKA-II) and Lens culinaris agglutinin-A AFP (AFP-L3) have been reported to represent highly specific tumor markers when compared with AFP for the early detection of HCC (8,9).…”
