Usefulness of dynamic urethral resistance relation (DURR) measurement for differential diagnosis between static and dynamic urinary obstruction in male spinal cord injury patients

Abstract: Pattern A showed a sensitivity with respect to the dynamic obstruction of 79% and a specificity of 100%. The sensitivity of pattern B with respect to the diagnosis of static obstruction was 71% and the specificity was 100%.

“…For example, in patients with low detrusor pressure and weak flow rate, AG number and LinPURR analysis would not be very accurate, mainly because of defects in the methods of calculation. [ 8 9 10 11 12 ] Because AG number and LinPURR are based mainly on maximum flow rate and corresponding detrusor pressure, they are calculated at a certain time point during voiding. However, the remainder of the urination process is neglected.…”

“…Because urodynamic parameters measured in men were used as reference indicators for work capacity parameters,[ 1 2 4 5 ] we omitted men with low detrusor pressure (<40 cmH 2 O) to avoid its interference with AG number and LinPURR analysis. [ 8 9 10 11 12 ] We also measured WIV parameters in a number of age-matched women. Because the number of cases was small, and no BOO diagnostic criteria in women are generally accepted, we simply compared WIV parameters of women with those of the men and did not carry out more complex analysis.…”

“…This ICI‐RS committee has collected and discussed the evidence with regard to female and male voiding dysfunction. Whereas ‘static’ bladder outlet obstruction voiding dysfunction, as a consequence of prostate enlargement in men, is well defined and consensus exists about testing, agreement about definition and quantification of ‘dynamic’ outlet dysfunction, in (younger) men or women the situation is less well defined or accepted . The ICI‐RS Think Tank chose to discuss especially urethral and pelvic floor function and functional tests (whether or not introduced for analysis of continence function); however, imaging is also included in the review tables and mentioned briefly in some parts of this report.…”
