ResumoIntrodução: A mucosa intestinal inflamada contém um grande número de neutrófilos e proteínas fecais derivadas dessas células, como a alfa-1-antitripsina, calprotectina, lactoferrina, elastase de polimorfonucleares, lisozima, entre outras moléculas, que são marcadas e estudadas como indicadores de inflamação do trato gastrointestinal. Objetivo: Mostrar as evidências científicas relacionadas aos biomarcadores inflamatórios fecais utilizados na prática médica, para avaliação da atividade doença e diagnóstico das doenças inflamatórias intestinais. Material e Métodos: Foram avaliados artigos das bases de dados BVS, SciELO, PubMed e Science Direct. Os descritores utilizados nesta revisão foram "inflammatory bowel disease" AND ("biological markers" OR "fecal biomarkers"). Foram considerados manuscritos publicados entre 2011 e junho de 2015, disponíveis na íntegra e publicados em língua inglesa. Resultados: Inicialmente foram identificados 184 artigos. A primeira seleção resultou em 44 artigos. A leitura e análise completas dos artigos resultaram em uma amostra final de 11 manuscritos. Verificou-se que a calprotectina esteve presente em todos os artigos revisados. Apenas um manuscrito avaliou o perfil de atividade da lactoferrina e nenhum artigo foi identificado que contemplasse os marcadores alfa-1-antitripsina e elastase de polimorfonucleares nas DII. Conclusão: Os achados deste estudo apresentaram a relação significativa dos biomarcadores fecais com o diagnóstico das doenças inflamatórias intestinais e o papel fundamental da calprotectina no estudo, diagnóstico e avaliação de recorrência pós-operatória. Abstract Introduction:The inflamed intestinal mucosa contains a large amount of neutrophils and fecal proteins derived from these cells, such as alpha-1-antitrypsin, calprotectin, lactoferrin, polymorphonuclear elastase, lysozyme, and other molecules that have been selected and studied as indicators of inflammation gastrointestinal tract. Objective: Show the scientific evidence related to the fecal inflammatory biomarkers used in medical practice for evaluation of the activity and diagnosis of inflammatory bowel disease. Materials and Methods: We searched the electronic databases of BVS, SciELO, PubMed, and Science Direct for a period spanning 2011 and June 2015. The descriptors used in this review were "inflammatory bowel disease" AND ("biological markers" OR "fecal biomarkers"). We considered full-text articles in English. Results: Initially, we identified 184 articles. The first selection resulted in 44 articles. After a complete reading and analysis of the articles, our final sample resulted in 11 articles. The calprotectin was found in all articles. Only one article evaluated the activity of lactoferrin profile. Gastrointestinal symptoms such as abdominal pain, dyspepsia, and diarrhea are relatively nonspecific and a common cause for seeking medical attention. To date, it is challenging for physicians to differentiate between functional and organic gastrointestinal conditions and it involves the use of serological and endoscopic techniques. Therefore, a simple, noninvasive, inexpensive, and effective test would be of utmost importance in clinical practice. Fecal calprotectin (FC) is considered to be a reliable biomarker that fulfills these criteria. FC can detect intestinal inflammation, and its level correlates well with macroscopic and histological inflammation as detected by colonoscopy and biopsies, respectively. FC has a decent diagnostic accuracy for differentiating organic diseases and functional disorders because of its excellent negative predictive value in ruling out inflammatory bowel disease (IBD) in symptomatic undiagnosed patients. There is accumulating evidence that FC has been effectively used to monitor the natural course of IBD, to predict relapse, and to see the response to treatment. This novel biomarker has the ability to assess mucosal healing (MH), which is a therapeutic goal in IBD management. A literature search was carried out using PubMed with the keywords FC, IBD, intestinal inflammation, and MH. In our review, we provide an overview of the utility and scope of FC as a biomarker in patients with IBD as well as undiagnosed patients with lower gastrointestinal symptoms.

Abstract: Our simple and rapid assay system has excellent performance for assessing mucosal inflammation of IBDs and can be replaced for ELISA. Practical application of this assay system enables us to use FC measurement more widely in clinical practice.
