Useful maneuvers for precise laparoscopic liver resection

Abstract: Laparoscopic liver resection has evolved over the past decade and nearly replaced open exploration. This procedure not only provides comparable oncological outcomes, but it also has improved recovery after surgery. Many of the challenges presented by limitations of instruments and techniques have been overcome through adaptations and new developments, and it is possible that the remaining obstacles will be overcome within a few years. Moreover, as surgeons continue to gain experience, their technical knowledge… Show more

“…For laparoscopic selective Glissonian pedicle approach, owing to movement limitations of basic instruments, some special instruments are required, for instance, Goldfinger dissector (Johnson & Johnson, Le Locle, Switzerland), Endo Retract Maxi 10 mm (Covidien, Tokyo, Japan) and Endo Mini-Retract 5 mm (Covidien), etc. [ 5 ]. Among these, endo mini-retractor is the most available one and, moreover, its size is the most appropriate for encircling the second and third order pedicles.…”

“…Among these, endo mini-retractor is the most available one and, moreover, its size is the most appropriate for encircling the second and third order pedicles. Despite an aid of these special instruments, the dissection has to be performed blindly at the posterior side, leads to some injuries and bleeding [ 5 ]. Our technique could solve this issue, as it simply provides non-traumatic tunneling, because the softness of the urinary catheter makes the tip of the retractor blunt.…”

“…It is difficult to fully grasp the anatomical relationship between the lesion and the vessel, as well as the amount of pre-resected liver and the resection boundary, and it is impossible to develop the optimal surgical plan. Therefore, traditional hepatectomy may increase the possibility of vascular and bile duct damage to patients, and increase the incidence of surgical complications in patients (6,19,20). Precise liver resection is a surgical plan based on precise preoperative assessments for different cases, and is combined with new technologies (2,5).…”

“…LLR was first introduced in 1992 and is increasingly used due to new technologies and devices [7]. Compared to open liver resection, LLR has several advantages including decreased blood loss, shorter hospital stay, and improved cosmetic results.…”
