Use of tracheal stenting in the palliation of anaplastic thyroid carcinoma: tertiary centre experience

Abstract: Tracheal stenting is a relatively safe and effective method for palliation of distressing airway symptoms in patients with anaplastic thyroid carcinoma. Early prophylactic tracheal stenting in anaplastic thyroid carcinoma may be an effective option to prevent development of airway compromise as the disease progresses.

“…Although an association between controlled motivation and poorer outcome would be consistent with predictions based on Self-Determination Theory ( Ryan & Deci, 2008), no such association has been found in our study nor in the available eating-disorder literature. One explanation for the lack of effect of controlled motivation on trajectories of eatingdisorder symptoms may be that more constraining interventions (such as hospitalization or family-based therapy) have been shown to be effective (Collin, Power, Karatzias, Grierson, & Yellowlees, 2010;Le Grange, Lock, Agras, Bryson, & Jo, 2015;Lock et al, 2010;Madden et al, 2015), and may over-ride effects of controlled motivation.…”

“…7) Unlike most thyroid cancers, which are generally well-differentiated and slow-growing, anaplastic cancer and malignant lymphomas can grow rapidly and cause tracheal compression; emergent tracheostomy or early thyroidectomy may therefore be required to secure the airway. 3,8) The case presented here also showed rapid deterioration within 2-3 months. In terms of diagnosis, fine needle aspiration is often used to evaluate the thyroid mass, although the diagnostic accuracy is only approximately 60% due to histological similarities to Hashimoto's thyroiditis.…”

“…In these cases, cordectomy or tracheostomy is required 2 . Further studies are also required to clarify the potential benefit of stenting prophylactically, before airway symptoms develop, 3 as the procedure may be technically easier with a greater intraluminal diameter when there is no emergent airway concern.…”

“…From the studies included in this review, it appears that treating locally advanced thyroid cancer with a stent does not negatively impact mortality. Two studies compared the survival of a stented and non‐stented cohort with the aggressive ATC, where airway obstruction is common and rapid 2,3 . Both studies report similar survival rates between groups but are limited by heterogeneity and significant bias.…”
