Background: While chronic dialysis treatment has been suggested to increase pulmonary pressure values, right ventricular dysfunction (RVD) is a major cause of death in patients with end-stage renal disease. We investigated the impact of different dialysis treatments on right ventricular function. Methods: We examined 220 subjects grouped as follows: healthy controls (n = 100), peritoneal dialysis (PD; n = 26), hemodialysis (HD) with radial arteriovenous fistula (AVF; n = 62), and HD with brachial AVF (n = 32). Echocardiography including tissue Doppler imaging (TDI) of the right ventricle was performed in all patients. Results: Pulmonary pressure values progressively rose from controls across the 3 dialysis groups (21.7 ± 6.8, 29.7 ± 6.7, 37.9 ± 6.7 and 40.8 ± 6.6 mm Hg, respectively; p < 0.001). TDI indices of right ventricular function were more impaired in HD patients, particularly in those with brachial AVF. RVD, assessed by TDI myocardial performance index, was higher in HD patients compared with PD patients (71.3 vs. 34.6%, p < 0.001). Moreover, the prevalence of RVD further increased in patients with brachial AVF compared with the radial access (90.6 vs. 61.3%, p < 0.001). Conclusions: Compared to DP, HD increases the risk of RVD, particularly in the presence of brachial AVF. TDI may detect early functional failure of the right ventricle in HD patients.

Abstract:Purpose: Heart is aff ected structurally and functionally in end-stage renal disease (ESRD). However, the data available about adverse eff ects of ESRD on right ventricle (RV) is scarce. We aimed to evaluate echocardiographic parameters of RV in nondiabetic, normotensive patients with ESRD undergoing hemodialysis (HD). Methods: A total of 45 (24 women; mean age 52.4 ± 12.4 years) consecutive nondiabetic, normotensive patients with ESRD undergoing HD and 39 healthy age and sex-matched control subjects (22 women; mean age 50.3 ± 6.6 years) were enrolled in the study. M-mode and two dimensional images, color, pulsed and continuous wave Doppler, and tissue Doppler measurements were acquired from all subjects. Echocardiographic evaluation was performed in the days between HD dates of the patients. Results: RV fractional area change, tricuspid annular plane systolic excursion, tricuspid E velocity, E/A ratio, tricuspid annular E´ velocity, and E´/A´ ratio were lower in patients than controls (p < 0.001, p = 0.003, p = 0.007, p = 0.005, p < 0.001, and p = 0.034, respectively). However, RV diastolic area, RV myocardial performance index, E/E´ ratio, and mean and systolic pulmonary artery pressure were higher in patients than controls (p < 0.001, p = 0.007, p = 0.005, p < 0.001, p = 0.006, respectively). Conclusions: RV systolic and diastolic functions of nondiabetic, normotensive HD patients are deteriorated as compared to healthy controls.

Myocardial diastolic dysfunction is the relaxation abnormality of ventricles that limits the diastolic fi lling and generally precedes diastolic heart failure. Diastolic dysfunction is a common fi nding in diabetes. Diabetic patients receive hyperbaric oxygen (HBO) therapy for non-healing lower extremity ulcers, and exposure to HBO therapy is known to infl uence cardiovascular functions. This study was designed to evaluate the effect of HBO therapy on myocardial diastolic function in diabetic patients. Thirty diabetic patients (18 male and 12 female, 59.9 ± 10 years old), who were planning to undergo HBO therapy, were consecutively enrolled. Myocardial diastolic function was evaluated by pulsed wave Doppler echocardiography and tissue Doppler echocardiography before the fi rst HBO therapy and after the tenth HBO therapy session. HBO therapy improved the relaxation capability of left ventricular myocardium, which was refl ected by reduction in E wave deceleration time of mitral valve infl ow (286.1 ± 65.8 msec vs 214.3 ± 32.1 msec, p < 0.05). HBO therapy also affected favorably the diastolic fi lling dynamics of right ventricle, which was partially refl ected by the changes in E wave peak velocity of tricuspid valve infl ow (0.48 ± 0.07 m/sec vs 0.46 ± 0.09 m/sec, p < 0.05). Tissue Doppler parameters of mitral lateral annulus, which are better correlated with ventricular relaxation, tended to be improved after HBO therapy, but the degree of improvement was not statistically signifi cant. In conclusion, we suggest that HBO therapy may improve the myocardial diastolic function of diabetic patients when applied repetitively. echocardiography; tissue Doppler; diabetes mellitus; hyperbaric hyperoxia; diastolic dysfunction.
