Fluoroquinolones are the most promising anti-infective chemotherapy agents depicting broad spectrum and potent activity. Various substituted fluoroquinolones derivatives were evaluated against gram negative, gram positive and resistance bacteria strains. These drugs are having DNA gyrase and growth inhibition activity. They have a relatively simple molecular nucleus, which is willing to many structural modifications. These agents have several favorable properties such as good bioavailability, tissue penetrability and relatively low incidence of adverse and toxic effects. They are effective in treatment of various infectious diseases. This paper is an attempt to the therapeutic prospects of fluoroquinolones as antibacterial agents.

“…At present, the most commonly used FQs in the United States are ciprofloxacin, levofloxacin, gatifloxacin, and moxifloxacin. 72 Ciprofloxacin has been approved for simple and complex urinary tract infections (including cystitis, pyelonephritis and chronic bacterial prostatitis, genitourinary and rectal gonorrhea without complications), skin and other soft tissue infections, bone and joint infections, infectious diarrhea and typhoid, intra-abdominal infections (when used with metronidazole), sinusitis and hospital-acquired pneumonia, etc. Ciprofloxacin is also approved for empirical treatment in patients with febrile neutropenia, prevention and treatment of anthrax, and lower respiratory tract infections (including acute bacterial deterioration of chronic bronchitis, pneumonia (except pneumococcal pneumonia ), hospital-acquired pneumonia, etc).…”

The clinical application of quinolone antibiotics is particularly extensive. In addition to their high efficiency in infectious diseases, the treatment process brings multiple hidden dangers or side effects. In this regard, drug resistance becomes a major challenge and is almost unavoidable in the clinical application of quinolones. Both genetic and phenotypic variations contribute to bacterial survival resistance under antibiotic therapy. This review is focusing on the drug discovery history, compound structure, and bactericidal mechanism of quinolone antibiotics. Recent studies bring a more in-depth insight into the research progress of quinolone antibiotics in the causes of death, drug resistance formation, and closely related SOS response after disease treatment at this stage. Combined with the latest clinical studies, we summarize the clinical application of quinolone antibiotics and further lay a theoretical foundation for the mechanism study of resistant or sensitive bacteria in response to quinolone treatment.

The quinolone class of antimicrobial agents has generated considerable interest since its discovery >40 years ago. Substantial progress has been made in our understanding of the molecular mechanisms of the action of quinolones against pathogenic bacteria, the induction of resistance to quinolones in these organisms, and the potential of each quinolone compound to induce toxicity in treated patients. Here, these key discoveries are reviewed; the present indications approved by regulatory agencies are described in detail, with comments on adverse events caused by quinolones in treated patients; and speculation about the future of the quinolones is proffered, even though their future is difficult to predict, because many factors may affect their clinical usefulness. However, the emergence of bacterial resistance to the quinolones is a major factor that will determine the future clinical effectiveness of these agents, so that intense investigation of mechanisms to either prevent or curtail resistance to quinolones is of prime importance to their future.
