Use of the ProSeal™ laryngeal mask airway in a pregnant patient with a difficult airway during electroconvulsive therapy

Abstract: We describe a patient at 20-22 weeks gestation, with a known difficult airway, who underwent eight sessions of electroconvulsive therapy using the ProSeal laryngeal mask airway and controlled ventilation. The airway management options for brief periods of general anaesthesia in patients with increased gastric volume are discussed.

“…It has been successfully placed even in obese patients either in anaesthetic or critical care situations. [7891011] There are various described techniques of insertion of PLMA. [12] Guided insertion was more frequently successful than the digital and IT techniques in patients with simulated difficult airway with a rigid neck collar.…”

“…82 ELECTIVE CASES OF KNOWN DIFFICULT AIRWAY There are four reports of use of the PLMA to overcome known airway difficulties. [82][83][84][85] In three cases the PLMA was used instead of tracheal intubation. In one case the PLMA was placed awake, using topical anesthesia.…”

“…[14] Ivascu Brown et al described the use of ProSeal laryngeal mask airway (PLMA) and controlled ventilation for eight sessions of electroconvulsive therapy in a pregnant patient in 2 nd trimester of pregnancy. [15] CONCLUSION: We conclude that the LMA is effective and safe for in carefully selected ASA 1 pregnant patients in the hands of experienced Anesthesiologist. The soft and rounded edge of LMA gives better compliance to a spontaneously breathing patient and at the same time prevents the injury to the soft tissues.…”
