The aim of this study was to evaluate the rhombic three-dimensional plate in fixation of displaced low subcondylar mandibular fractures clinically and radiographically. MATERIALS AND METHODS: This is a prospective, interventional, single armed case series study that was carried out on twelve patients with displaced low subcondylar mandibular fracture. Open reduction and internal fixation was utilized to treat those fractures using rhombic 3D plate, patients were collected from the department of Oral and Maxillofacial surgery, Faculty of Dentistry, Tanta University. All cases were treated with retromandibular approach. Post-operative evaluation: all patients underwent regular follow up for six months. The following parameters were evaluated: a. Maximal mouth opening, b. Lateral and protrusive mandibular movements, c. Chewing and occlusion, d. Clicking and tenderness of TMJ, e. Vertical height of the ramus, f. Anteroposterior angulation of the condyle. RESULTS: 6 months after the operation, the functional parameters returned to normal, with an average mouth opening of 43.6 mm, protrusion of 9.6 mm, and laterotrusion of 10.5 mm. No clicks or tender TMJ, normal function of facial nerve. Radiographic controls showed good fracture alignment, no plate fracture, bending or loose screws were observed. CONCLUSIONS: The clinical and radiographical data suggested that the rhombic three-dimensional plate is suitable for treatment of condylar fractures and provided stable fixation.

ZusammenfassungAuch wenn sie in Standardsituationen heute selten sind, stellen Pseudarthrosen eine relevante Komplikation dar. Insbesondere nach aufwendigen Kieferrekonstruktionen mittels z. B. Fibula- und Beckenkammtransplantaten sind Komplikationen mit hohen Belastungen für die betroffenen Patienten und die erbringende chirurgische Disziplin verbunden. Dieser Beitrag zeigt, wo und warum Pseudarthrosen in der MKG-Chirurgie heute eine Rolle spielen und welche Therapieoptionen bestehen.

The aim of this review was to present and discuss current views on the treatment of condylar fractures (CFs). The authors addressed the following issues: the etiology, epidemiology and mechanisms of CFs; strategies and methods for the treatment of CFs. Moreover, the choice of surgical approach for the open treatment of CFs as well as techniques and materials used for fixation of CF are discussed.The PubMed database was used to search for relevant articles published between 2000 and 2018. The analysis referred to both original and review papers (including meta-analyses) that concerned adult patients. There are still differences in opinions among researchers regarding the choice of appropriate treatment – closed or open. There is no consensus among open treatment supporters in choosing the right surgical approach. The important question is which material to choose for osteosynthesis of the condylar fracture.Recent studies show satisfactory results in terms of stability of condylar osteosynthesis with the use of 3D plates. Further discussion is required on the choice of material for fixation of the condylar fracture and, in particular, on the possibility of using resorbable materials.
