Use of the AAEM Guidelines in Electrodiagnosis of Ulnar Neuropathy at the Elbow

Abstract: Thibault MW, Robinson LR, Franklin G, Fulton-Kehoe D: Use of the AAEM guidelines in electrodiagnosis of ulnar neuropathy at the elbow. Am J Phys Med Rehabil 2005;84:267-273.Objective: The goal of this study was to determine if practice patterns in Washington State were altered by the development and publication of the American Association of Electrodiagnostic Medicine practice parameter for electrodiagnosis of ulnar neuropathy at the elbow.Design: Medical records from 350 workers' compensation patients who und… Show more

“…However, to the extent that historical providers of this service represent a performance benchmark, such outcomes have not been evaluated for electrodiagnostic testing by neurologists and PM&R physicians. 28,44 This study has several implications for clinical practice. First, about one-third of patients with diabetes and presumed DPN may not have a large fiber polyneuropathy or may have focal pathology such as lumbosacral radiculopathy.…”

“…Due to the ulnar nerve’s superficial position at the elbow, ESKD patients may be predisposed to ulnar nerve compression particularly if the elbow is rested on a hard surface during dialysis. NCS demonstrate slowing of ulnar nerve conduction across the elbow, which may be accompanied by reductions in motor and sensory amplitudes (98). In cases where there is prominent motor involvement, characterized by weakness and atrophy, electromyography may demonstrate denervation in distal muscles.…”
