Use of Preventive Aspirin Among Older US Adults With and Without Diabetes

Abstract: IMPORTANCEThe net benefit of aspirin for prevention of cardiovascular disease (CVD), particularly primary prevention, remains debated in people with and without diabetes. Recent studies suggest that the benefits of preventive aspirin may be outweighed by the potential for harm in older adults; therefore, it is important to monitor current aspirin use in order to minimize risk for future harm in the oldest segment of the population.OBJECTIVE To determine the prevalence of preventive aspirin use in older US adul… Show more

“…However, the unwarranted use of aspirin was found to be low as only 5.1% were using aspirin upon recommendation of a non-physician such as family or friends. These findings are consistent with a paper written by Liu et al [ 11 ]. Based on their accounts, 61.7% of older American patients with diabetes mellitus (DM) were aspirin users whereas 42.2% of those without DM were also reported to be taking the medication.…”

“…In Denmark [ 14 ], aspirin use was independently associated with age, however, researchers pointed out that due to the widespread use of aspirin including the concomitant use of other medications specifically among older adults, the incidence of major bleeding became an issue suggesting that appropriate measures should be in place to limit the misuse of aspirin. Contradicting these reports, a study conducted among Chinese patients [ 11 ], found that among patients with diabetes, the likelihood of aspirin use did not differ significantly by age, and the level of cholesterol was also negatively associated with aspirin use while patients in low-level hospitals were more likely to exhibit inappropriate use of the medication.…”

“…The foremost consideration for the appropriate use of drugs in a preventive manner would be the strict adherence to the recent changes in clinical guidelines. For example, from 2011–2018, the prevalence of aspirin use among older US adults (≥60 years) was high among individuals at an increased risk for bleeding and other adverse events [ 16 ], despite recent changes in the guidelines of the American Heart Association, American Diabetes Association and USPSTF. Moreover, other considerations should be considered before the initiation or continuation of preventive medications use, such as age, multimorbidity, number of other medications and updated guidelines.…”

“…For a long time, people who have one or more risk factors for cardiovascular disease (CVD) have been recommended to take aspirin, one of the most often used drugs, to prevent CVD ( Ittaman et al, 2014 ). According to estimates, 46.7% of US individuals aged 60 or older reported using aspirin to prevent CVD events ( Liu et al, 2021 ). However, no consensus was established between the various guidelines committees on whether or how to use aspirin for primary prevention ( Patrono and Baigent, 2019 ).…”
